This post supports the Lead Directorate Pharmacist for Medicine in providing a clinical service to the Renal department including transplant, anaemia and dialysis and speciality ward cover. The post holder provides advice to the renal department, and works on behalf of pharmacy to lead on renal management. The pharmacist works alongside other members of the multidisciplinary renal and medicine team, including consultants, junior doctors and a range of specialist nurses in areas such as transplant, peritoneal and haemodialysis, advancement into renal replacement therapy and renal anaemia, specialist pharmacists in acute medicine and emergency admissions whilst liaising with the renal outpatient team. To provide a clinical pharmacy service to the Renal Speciality within the Medicine Division at RCHT. To provide specialist pharmacy advice and training in the areas of renal, medicines management and optimisation to consultants & junior medical staff, nursing staff, pharmacy staff, other healthcare professionals, & patients/carers. To assist in the provision of regular structured feedback to renal regarding the safe and effective use of medicines and drug expenditure to facilitate service improvement. To undertake and contribute to clinical audit and quality improvement within Pharmacy and the Speciality To support the General Medicine team across professional and organisational boundaries About us
