Job summary
An opportunity has arisen for an enthusiastic and highly motivated DSN to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. This mobile service will also comprise a Dietician, Clinical psychologist, Youth Worker, Social Prescriber, and administrator.

This exciting new post has been developed to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. The post holder will support young adults who may be struggling with a range of difficulties related to living with diabetes, as they make the transition to adult services. The work will be with young adults living with both Type 1 and Type 2 diabetes.

The post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users.The post will have a base at Mile End Hospital, but the job will involve working across all of the Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), and ensuring that standardised, high-quality care is delivered across the Trust.

In addition to being a member of the core young adult MDT the successful candidate will join the wider Barts Health diabetes service and will be well supported in this highly regarded service, by more senior clinicians.

Main duties of the job
The post is an exciting opportunity to join an innovative service that aims to improve diabetes care and outcome in young people with diabetes (both type 1 and type 2) across Barts Health. The post holder will be part of a core Young Adults diabetes team that will support transition and young adult diabetes services across Bart's health.

The post holder's primary role will be supporting high risk young adults and working with the team's social worker, youth worker and psychologist to safeguard these patients and engage them.

The post holder will develop specialist skills enabling them to provide management plans, treatment advice, and support for young adults (age 16-25) with diabetes and their carers.

The post holder will work across all sites providing specialist care and support for these young people, their families and the site based clinical diabetes teams.

The post holder will be a clinical and educational resource for patients and health professionals.

The post holder will be a good team player always striving to improve the care of people with diabetes.

Qualifications
Essential

• First level NMC registration
• Mentorship qualification
• University diabetes course or equivalent level of knowledge or experience

Desirable

• Teaching and assessing (ENB998) or equivalent level of knowledge or experience
• Graduate in nursing or health related studies
• Recognised certificate in counselling

Experience
Essential

• Post-registration experience
• Experience of working with young people with diabetes
• Experience of supervision of junior staff / nursing students
• Experience of patient education to support self-management
• Experience working with diabetes technology

Desirable

• Participation in Nursing Research and Audit
• Experience in the management of diabetes in the community or primary care or of working on a diabetes ward

Knowledge

• Awareness of the structure and organisation of the wider NHS
