An opportunity has arisen for an enthusiastic and highly motivated DSN to join the innovative Barts Health NHS Trust Young Adults Diabetes Service. This mobile service will also comprise a Dietician, Clinical psychologist, Youth Worker, Social Prescriber, and administrator. This exciting new post has been developed to improve diabetes care and outcomes for young people (age 16-25) with diabetes across North East London. The post holder will support young adults who may be struggling with a range of difficulties related to living with diabetes, as they make the transition to adult services. The work will be with young adults living with both Type 1 and Type 2 diabetes. The post holder will have scope to develop the service, with the support of more senior colleagues and in collaboration with service users.The post will have a base at Mile End Hospital, but the job will involve working across all of the Barts Health sites (Whipps Cross Hospital, Newham University Hospital, the Royal London Hospital, and St Barts Hospital), and ensuring that standardised, high-quality care is delivered across the Trust. We welcome applications from qualified diabetes specialist nurses with a strong interest in working with this patient group. In addition to being a member of the core young adult MDT the successful candidate will join the wider Barts Health diabetes service and will be well supported in this highly regarded service, by more senior clinicians.
