What you will do:

* Manage a complex caseload of patients within Intermediate Care beds, delivering evidence-based occupational therapy assessments and interventions.
* Complete functional, cognitive, environmental, and moving and handling assessments to support rehabilitation and discharge planning.
* Undertake home visits to assess suitability for discharge, identify risks, and recommend equipment, adaptations and support services.
* Work collaboratively within the multidisciplinary team to develop and review rehabilitation goals and discharge pathways.

Person Specification:

Qualifications:

* Degree or Diploma in Occupational Therapy.
* Current registration with the Health and Care Professions Council (HCPC).
* Evidence of ongoing Continuing Professional Development (CPD).

Experience:

* Previous Band 5/6 Occupational Therapy experience within Intermediate Care, Community Rehabilitation, Discharge Planning, Acute Medicine, Elderly Care or Integrated Community Services.
