We are currently seeking an experienced

Community Mental Health Nurse, at Band 6 to work via a Bank Contract, based in Hemel for the Specialist Mental Health Teams for Older People (SMHTOP) North West Community Team. You will be joining our Temporary Staffing Team within HPFT - Hertfordshire Partnership University NHS Foundation Trust.

You will be required to take responsibility for the assessment of care needs, the development, implementation, and evaluation of programmes of care and the setting of standards of care and management of a defined caseload, including liaison with other agencies and, where appropriate, the supervision, development and teaching of staff and/or students. You will act as care co-ordinator to a case load of service users, ensuring the care plan is always met, and to work as a member of the multi-disciplinary team.

As a staff member, you are representatives of HPFT and play a vital role in fostering positive relationships and communication between the Mental Health Trust and not only the service users and their carers and relatives, but also other agencies, charities, and providers.

All applicants need to hold a valid Professional Registration - NMC - and can evidence this.

You will be responsible for the full assessment of care needs and risk assessment of service users presenting with a wide variety of clinical conditions.

Following this, the post holder will be responsible for the development, implementation, and evaluation of programmes of care, without direct supervision (though clinical supervision is implemented as per policy to support you).

You will manage oneself in a professional manner, responding to requests, enquiries etc in a timely fashion, and respond to any crisis that a service user may advise of, with the aim of promoting independent living in the community

The post holder will work with the philosophy to support service users to remain in their own home.

Hertfordshire Partnership University NHS Foundation Trust (HPFT) is rated by the Care Quality Commission as an Outstanding provider of mental health and learning disability services. Feedback from our staff about working for the Trust is very positive and, in the NHS national staff survey, we regularly see a high percentage of staff saying their role makes a difference to our service users and that they would recommend the Trust as a place to work. Our people tell us they feel supported though a great development, wellbeing and work-life balance offer and a highly compassionate, values-driven culture. We are equally proud of our staff, who live our values of being welcoming, kind, positive, respectful and professional so that together we provide great care and great outcomes for our service users and carers.
