About the Role

We are seeking an enthusiastic and experienced Social Worker to join our integrated team, focusing on the transition and provision of support for young adults with disabilities (aged 16+) moving into and utilising adult services.

This vital role involves statutory duties under relevant Scottish legislation, promoting well-being, choice, and control for service users and their carers.

Key Responsibilities

• Assessment & Care Planning: Conduct comprehensive needs assessments (including Single Shared Assessment) and risk assessments for young adults and adults with physical and learning disabilities. Develop and review person-centred support and care plans.
• Transition Support: Lead and coordinate the transition process for young people moving from children's to adult services, ensuring a smooth and successful experience.
• Safeguarding: Carry out safeguarding duties, including adult protection investigations, in line with local procedures and legislation.
• Case Management: Manage a complex caseload, utilising effective planning, intervention, and review strategies.
• Resource Allocation: Identify and arrange appropriate support services, personal budgets, and community resources.
• Multi-Agency Working: Collaborate effectively with families, carers, health professionals, housing, education, and third-sector organisations.
• Legal Compliance: Ensure all practice is compliant with relevant legislation, including the Social Work (Scotland) Act 1968 and the Adults with Incapacity (Scotland) Act 2000.

Qualifications and Skills:

• A degree in Social Work or an equivalent qualification recognised by the Scottish Social Services Council (SSSC).
• Current SSSC registration as a social worker.
