The Purpose of the post is to support children and young people in care and care leavers to achieve the best possible outcomes and make a successful transition into adulthood The Objectives are
1. To support the implementation of children & young people’s care and pathway plans
2. To prepare young people for a successful transition to adulthood
3. To support care leavers to move towards independence
