The Structural Heart Valve Disease (SHVD Enhanced Practitioner (EP) in this position is responsible for the care coordination/ triage and patient facilitation of of the structural heart cardiology patient pathway as part of a team of structural heart Enhanced practitioners and consultant cardiologist. Responsibilities include Transcatheter Aortic Valve Implant (TAVI) and Transcatheter Edge to Edge (Mitral and Tricuspid) and all structural heart patient care coordination. The post holder will be required to work 8-16:00 or 08:00-18:00 shifts Monday to Friday, with some consideration of compressed hours and occasional Working from home The essential requirements that will be expected from the post holder is to review the process of patient referrals ensure the delivery of early responsive, high quality patient care. Having a patient centred focus to facilitate clinical assessment, admission, and safe discharge of inpatients. Program initiatives around improving patient experience, process, education and outcome appropriateness. The post will be considered for band 6, if successful applicant requires additional skills Main duties of the job
Lead SHVD to provide professionals with a succinct referral and triage route for patients requiring SHVD
Reviewing current referral pathways, evaluate and guide change.
Evaluate the patient's referral information provided by the referrer
Support clinical decisions of patients and in alignment with ESC and NICE guidance to support patients to be triaged appropriately to TAVI and not requiring surgical intervention.
Link with the SHVD to discuss the referral and an appropriate action plan
Liaise with the referrer to agree next steps
Continually evaluate the coordination process.
Work closely with referrers to provide reason for referrals, patients symptoms, level of urgency whilst ensuring speedy access to investigations e.g. CT TAVI,
Support patients to prepare for pathway e.g. dentistry,
Take opportunities to form networks for clinical developments locally and nationally.
Provide staff training of new ways of working
Clinical development to show benefits for investment and a sustainable patient pathway that works across Bristol with GP, and with District general and tertiary referrals About us

- Facilitate succinct communication between cardiologist / SHVD and referring physician regarding patient pathway position
- Facilitate communication and /or education/ guidance between primary care to support early referrals that promote early referral and address health inequality
- Coordinate any additional testing i.e. Cath, TTE, CT, if expected timelines expired.
- Coordinates concerns among multiple departments including but not limited to, Cath lab, anaesthesia, CT, echo, vascular, hospital units
- Facilitate scheduling and of structural heart procedures
- Act as clinical expert in assessing, discharging, escalation of care includig NMP and providing specialist knowledge and advice to nurses and clinical teams/staff working within Division and Trust.
- Review the information access that is available to patients and families through all stages of care and look for ways to support appropriate information sharing.
- Working with operational and outpatient teams to facilitated strong working relationships that support succinct referrers letters to SHVD team
- Support a SHVD wide approach on improvement, in conjunction with divisional education facilitators, to support triage of peri/post TAVI/Mitra Clip care to CCU/CICU and cardiac wards.
- Support the delivery and evaluation of TAVI/Mitra Clip pathway inclusive of investigations, treatment and procedures alongside digital content aligned to workforce priorities.
- Act as clinical expert providing specialist knowledge and advice to nurses and clinical teams/staff working within Division and Trust
- Dynamically support the SHVD team response to new service development demands such as maintaining patient pathways and promote practitioner led activities such as preoperative, follow up clinics and patient discharge.
- Ensure patient involvement activities and feedback are contemporary, and promote safe, outstanding patient care.
- Communicating with professional leads for nursing, midwifery and AHP and other members of staff across the Trust, including senior clinical staff.
- To develop and implement a personalised patient education content strategy for the SHVD service. This educational approach will include patients and carers knowledge and understanding of assessment, investigations and personalised information provision, all clearly documented to facilitate a synchronised care pathway.
- Review the next steps that support Cath lab competencies development progression to merge catheter lab and enhanced practitioner roles, that progress both staff and patient care.
- Initiates and coordinates education for primary care, GP and allied Health professionals
- Professionally demonstrate transformational and inspirational leadership with responsibility for supervising, leading and managing, as well as overseeing the practice development of the TAVI pathway and staff.
- Responsible for devising and providing training that is compliant of professional and regulatory standards such as NMC, HCPC, CQC and Health Education England and is cohesive and flexible with other CNS/EP/AP and clinical teams.
