North Tyneside Council – Older Persons Mental Health Social Work Team
We are looking for an experienced Social Worker who is passionate about working in an older persons mental health social work team. This is a role for a social worker who is confident, reflective, and values strengths-based social work.
The Older Persons Mental Health Social Work Team (OPMHSW) provides assessment and support to people, predominantly aged 65 years and over, who have mental health and social care needs. The team supports individuals living in the community who are experiencing a wide range of mental health conditions, ensuring that care is person-centred and tailored to individuals needs, strengths and desired outcomes.
We work alongside people experiencing a variety of mental health difficulties, including anxiety, depression, psychotic symptoms, memory problems and dementia. The team is based at North Tyneside General Hospital, Rake Lane, North Shields. We are co-located with our health colleagues and work closely as part of a multidisciplinary team.
Our service also supports individuals admitted to the psychiatric wards within North Tyneside General Hospital, and other wards including Campus of Ageing and Vitality (St Nicholas Hospital) and St Georges Park, Morpeth. Our role is to ensure that people receive appropriate social care assessment and support throughout their admission and to facilitate effective discharge planning and community support where required.
The OPMHSW team provides a professional and comprehensive social work service to older people, their carers and families across North Tyneside. Our aim is to deliver a high quality service that is responsive, accessible and effective in meeting the needs of older adults who have complex mental health and social care needs.
The multidisciplinary team consists of Consultant Psychiatrists, Social Workers, Community and ward-based Psychiatric Nurses, Psychologists and Occupational Therapists. The Younger Persons Dementia Service and Memory Clinic are also part of the wider Older Persons Mental Health Service. Working collaboratively enables us to provide coordinated, holistic care and ensure that individuals receive a tailored response unique to their outcomes.
As social workers within OPMHSW, our role involves listening to and supporting individuals to achieve positive outcomes. This may include assessing and addressing social care needs, facilitating safe and timely hospital discharge, arranging or reviewing care and support services and helping individuals maintain their independence and wellbeing within their communities. We are commited to adapting our practice to meet the unique needs, wishes and circumstances of individuals while promoting equality, inclusion and choice. We also undertake carers assessments and provide practical and emotional support to carers recognising the vital contribution they make in supporting their loved ones.
Our practice is underpinned by strengths-based approach which seeks to empower individuals, build resilience and promote independence. We ensure that peoples voices, wishes and desired outcomes remain central to all assessments, care planning and interventions.
We work effectively in partnership with a range of health, social care and voluntary sector organisations to ensure the best possible outcomes for the person and their families. Effective multidisciplinary and multi-agency working is fundamental to delivering coordinated, person-centred care and support.
