Job summary
30 hours per week - permanent.

There is an exciting opportunity to join, the Mental Health Assessment and Intervention Team. The service was set to provide high quality care for people experiencing mental health problems in Plymouth and has proved a success across the city. The service will offer evidence-based treatment pathways for people that require more support than can be offered in Primary Care Services and voluntary agencies within the community. The role will develop more effective links with primary care and third sector agencies to support those people.

Main duties of the job
We are seeking to recruit experienced and flexible with excellent networking skills to join the team. This post will offer a challenging and exciting opportunity to gain experience of working across Livewell's Core mental Health Service, working across a variety of settings such as GP surgeries, and Community hubs developing relationships with Primary Care, partner agencies and secondary mental health services. You will have the opportunity to deliver evidence-based recovery focussed mental health interventions.

The CMHS pathway provides a variety of opportunities for clinical support and development, including a tailored induction programme, regular multi-disciplinary team meetings, monthly individual supervision, and regular multi-disciplinary caseload supervision. As a member of the Core mental health teams, you will also contribute to service developments at the quarterly meetings.

Job responsibilities
JOB PURPOSE
The role will support the care of those patients who are:

Referred into the team by internal services, GP surgeries for support with their mental health, to provide signposting.

The role will offer mental state monitoring and risk assessment.

The role will provide a range of brief psychotherapeutic and social interventions, appropriate to the band of the role, for people experiencing mental health difficulties. via telephone or video call, occasionally face to face visits where this is deemed as the only appropriate medium.

The role involves working with people experiencing mental health concerns / complex emotional needs via a number of different means i.e. face-to-face, telephone and/or video call.

The role will also include an effective working knowledge of the voluntary sector as well as an understanding of the needs of carers and family.

The role will require working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within a multi-disciplinary team in delivering services.

There is a requirement for substantial periods of concentration and sitting and accurate and timely record keeping.

3. SCOPE AND RANGE
For people experiencing a mental health crisis, Video/Tele-coaches Intervention Practitioners will provide:

• Mental state assessments
• Clinical risk assessments
• Clinical formulations
• A range of recovery focussed interventions.
• Validation and de-escalation of crisis
• Distress tolerance and emotional regulation strategies
• Brief psychological interventions to people experiencing fluctuation in their mental health state.
• Consultation and advice to carers and families and other agencies to enable them to effectively support service users.
• Effective liaison with other parts of the mental health pathway and partner agencies
• Effective signposting and advice

4. MAIN DUTIES/RESPONSIBILITIES
Communication and Working Relationship Skills
Provide complex and sensitive information to a wide range of internal and external stakeholders (users of services, carers, multi-disciplinary staff, and other statutory / non statutory services).

Contribute to the safe and effective day to day operations of the service managing clinical enquires.

Provide a high standard of written and verbal communication that is clear and in line with local guidance.

Role model effective communication: demonstrating empathy, respect, compassion and hope where there may be barriers to understanding.

Communicate information in a way that makes it relevant and understandable in line with operational guidance e.g., SBAR.

Maintain professional relationships with all colleagues to ensure professional standards are met.

Provide consultation and advice to the service users support network.

Analytical and Judgement Skills
Undertake clinical risk assessments, based on latest empirical evidence and compliant with local policies / procedures.

Assist in Promotion of the Service locally and nationally as appropriate.

Act as the persons and their carers / familys advocate

Incident reporting - To report any incidents of harm or near miss in line with the Trusts incident reporting policy supporting appropriate actions are taken to reduce the risk of reoccurrence.

To attend Clinical Team Meetings and share relevant information with the clinical team.

Promote a recovery model that empowers persons, carers and relatives to be at the forefront of decision making, owning their packages of care / treatment.

Create, implement and update care plans and risk assessments as required.

Champion persons rights and service users including dignity, equality, diversity, choice and respect.

Provide advice and guidance regarding mental health problems.

Share clinical information on individuals coming into contact with Police Officers, as per agreed information sharing protocol under guidance from senior staff.

Request information on specific individuals from Police Officers as per information sharing protocol.

Promoting and safeguarding the welfare of children, young people and vulnerable adults

Implementation of NICE guidance and other statutory / best practice guidelines (if appropriate) under guidance from senior staff

To maintain up to date knowledge of legislation, national and local policies and issues in relation to service provision

Responsibility for Policy and Service Development Implementation
Participate in programmes of audit and research within the team as agreed, implementing the findings.

Contribute to clinical governance arrangements in order to ensure the quality of the service provided.

Involvement in the development of local systems and process that ensure the implementation of Trust policies.

To monitor waiting lists and ensure the team managers are aware of potential breeches in a timely manner.

Supervise and support junior staff with guidance from senior staff.

Monitor practice and development of junior staff as appropriate reporting to supervisory staff.

Demonstrate and role model excellent clinical leadership skills.

Ensure that core training is maintained.
