Job Title: Band 6 Physiotherapist - Elderly Care Team

What you will do * Independently manage a caseload within the Older Peoples Service and medical rehabilitation wards.
* Complete comprehensive assessments and develop tailored rehabilitation treatment programmes.
* Work closely with the multidisciplinary team to deliver coordinated, patient‑centred care.
* Support and supervise junior staff as required, delegating tasks appropriately.
* Develop and implement effective discharge plans to ensure continuity of care following discharge.
* Maintain accurate, timely, and high‑quality clinical documentation in line with service standards. Requirements * HCPC‑registered Physiotherapist.
* Experience working within elderly care, medical rehabilitation, or inpatient settings.
* Ability to work autonomously and manage complex caseloads.
