Lead Pharmacy Technician - Clinical Services

Job summary
This is an excellent opportunity to play a key role in delivering high-quality, patient-centred clinical pharmacy technician services within a supportive and forward-thinking multidisciplinary team.

Under the direction of the Principal Pharmacy Technician - Clinical Services, you will provide advanced clinical pharmacy technician support across designated wards and clinics, acting as the lead technician for a specialty or service area. You will be responsible for the delivery and continuous improvement of medicines optimisation processes, including medicines reconciliation, discharge facilitation and TTOs, patient counselling, and medicines supply pathways.

Working closely with pharmacists, nursing staff, medical teams, and other healthcare professionals, you will act as the technical point of contact for medicines-related processes within your specialty area, ensuring the safe, effective, and efficient use of medicines. You will contribute to service development, audit, quality improvement initiatives, and the implementation of best practice standards to enhance patient outcomes and reduce medicines-related risks.

Main duties of the job
As an experienced Clinical Pharmacy Technician, you will lead and coordinate medicines optimisation services across the Trust, ensuring patients receive safe, effective and timely pharmaceutical care throughout their hospital journey. Working closely with multidisciplinary teams, you will undertake medicines reconciliation, facilitate timely patient discharge, provide patient counselling, and support the safe use of high-risk medicines.

You will act as the lead technician for your specialty area, supporting service development, quality improvement and governance activities, while helping to deliver the Pharmacy Clinical Services vision under the direction of the Principal Pharmacy Technician - Clinical Services. The role includes supervising and developing pharmacy staff, contributing to rota management, training and competency assessment, and promoting a culture of continuous improvement.

The post holder will support audit, risk management and service improvement initiatives, ensuring compliance with Trust policies, medicines legislation and professional standards. You will also contribute to dispensary services, provide medicines-related advice to patients, and work collaboratively with pharmacy and clinical colleagues to improve patient outcomes, patient flow and the efficient use of medicines across the Trust.

Job responsibilities
  • Lead the technician contribution to medicines reconciliation at admission/transfer/discharge, ensuring accurate medication histories, allergy status, and documentation in EPR.
  • Proactively facilitate discharge (TTO accuracy checks within scope, supply coordination, patient counselling, liaison with primary care/community pharmacy). Liaise with patient flow and ensure that time critical discharges are processed as a priority to align with the trust strategy
  • Deliver targeted patient education training (e.g., inhaler technique, insulin administration aids, high-risk medicines counselling) and escalating pharmaceutical care issues to pharmacists as required.
  • Support safe use of high-risk and specialty medicines (e.g., anticoagulants, biologics) by coordinating pathways, supplies, monitoring schedules, training and documentation.
  • Act as the named clinical lead technician for the Pharmacy Technical Team Clinical Services, assist in coordinating technical workflows, standard work and competency across that area.
  • Co-ordinate the use of discharge carts at ward level, to ensure optimised flow though timely discharge at ward level.
  • Assist the Principal Pharmacy Technician in delivering the Pharmacy Senior Leadership Teams clinical service vision.
  • Perform ward-based final checks on the patients medication supplies, certify those suitable for supply on discharge and request re-labelling of medication that has changed or a supply of medication that is not available.
  • On admission identify current medication taken by patients and resolve problems identified in an appropriate manner
  • Identify patients with potential medication compliance/concordance problems and taking appropriate action

  • Professional registration as a Pharmacy Technician with the General Pharmaceutical Council, or relevant registration
  • NVQ Level 4 in Pharmacy Services/BTEC Accuracy checking qualification
  • Management qualification (or equivalent experiential learning achieved via experience of managing or supervising a team)
Experience

  • Experience of working in an NHS hospital pharmacy setting.

  • Diploma in Clinical Pharmacy Services and Therapeutics
