We have an exciting opportunity for a self-motivated, experienced Therapist to join our UCR Team in Oldham Community. We need people who share our excitement, embrace different ways of working and are able to work flexibly across organisational boundaries supporting the NHS Long Term Plan and admission avoidance agenda. Position
The post holder will work as part of the multi-disciplinary team, driving the changes to community care response to support the hospital avoidance agenda. They will work in line with the agreed strategic objectives of Northern Care Alliance. The aim of the team is to reduce unnecessary hospital admissions with an aim of avoiding attendance to the Emergency Department, by providing holistic therapy intervention and a person centred approach support in order to enable patients to remain in their own home or usual residence. The team works closely with community services to support the home first model. Main duties of the job
The successful candidate will be expected to work independently, but also as an integral member of the multidisciplinary team within the UCR team. The candidate will be able to demonstrate strategic planning and have a strong clinical background in Urgent Care or a similar field. Excellent time management and advanced communication skills are required along with awareness and understanding of local, regional and national agendas in respect to Health & Social care. About us

- Occupational Therapy or Physiotherapy Degree
- HCPC registration
- RCOT or CSP registered
- Previous experience within urgent / emergency care
- Experience of management and / or leadership
- Conduct comprehensive therapeutic assessment including assessing for mobility aids / stairs / equipment / dressing aids
- Ability to effectively discharge plan complex patients
- Able to manage own time effectively including planning and prioritisation of both clinical caseload and leadership responsibilities
- Experience of participating in clinical audit
- Engaged in clinical supervision
