The Hampshire and Southampton Learning Disability Service is seeking an enthusiastic and motivated Charge Nurse to join our Single Point of Access (SPoA) team, the gateway to our specialist service. This is an exciting opportunity to play a pivotal role in improving the health and wellbeing of adults with learning disabilities by ensuring they receive the right support, at the right time, from the right service. As a valued member of our innovative and forward-thinking SPoA team, you will work alongside a vibrant rotational multi-disciplinary team, undertaking comprehensive assessments of referrals and helping determine unmet health needs that may require specialist learning disability support. Across Hampshire, our community hospitals provide inpatient rehabilitation as a step down from acute care, and our dedicated teams also staff Treetops Sexual Assault Referral Centre in Portsmouth, offering expert, compassionate support. About the Role

- Communicating effectively with patients, families, carers and professionals.
- Working collaboratively with a wide range of internal and external partners.
- Supporting the delivery of high-quality, responsive and accessible learning disability services.
We are looking for a compassionate and skilled clinician who is
- A confident and experienced Learning Disability Nurse (other nursing fields will be considered with evidence of learning disability practice experience).
- An excellent communicator with strong interpersonal skills.
- Organised, adaptable and able to manage competing priorities.
- Experienced in assessment and clinical decision-making.
- Proficient in IT and administrative systems.
- Passionate about improving outcomes for people with learning disabilities.

- In depth experience of working with people with a learning disability, preferable within a community team setting.
- Excellent communication skills (written & verbal).
- Evidence of ability to work in a way which is flexible and responsive to the needs of individual service users.
- Evidence about clarity of the level of performance expected and how to support self and others to achieve this.
- Evidence of ability to change or adapt the way things are or explore opportunities change processes and work flexibly.
- Evidence of ability to build effective working relationships with other care and support providers outside the organisation.
- Evidence of effective team working and leadership skills.
- Ability to work autonomously and under pressure and set priorities for self and team staff
- Detailed knowledge of learning disability guidelines in current practice.
- Knowledge of the principles of clinical governance and its application.
- Ability to inject pace, energy and positivity into the work environment.
- Actively seeks and implements ideas for improving the quality of service from those working in their team, and the patients we serve.
- Ability to attend base daily when required is essential.
- Experience of RIO computer system.
- Evidence of championing service development.
- Coaching or leadership course.
