Health and Wellbeing Practitioner
Location: East Riding of Yorkshire
Salary : £26,000 per annum

We're looking for a passionate and motivated Health & Wellbeing Practitioner to join Forward Trust's Connections Team at HMP Millsike.
This is a full-time position working 35 hours per week, offering a unique opportunity to support people in custody to improve their physical, mental and emotional wellbeing, build resilience, and develop healthier lifestyles that can be sustained on release.
Working as part of our innovative Health & Wellbeing service, you'll collaborate with prison staff, healthcare professionals, peer mentors and external partners to create a positive, recovery-focused environment across the prison.
What You'll Be Doing
* Delivering 1:1 support, group interventions and wellbeing workshops
* Coaching and mentoring service users to achieve positive, lasting change
* Supporting and developing prisoner Health & Wellbeing Champions
* Promoting healthy lifestyles, recovery and wellbeing across the prison
* Organising health promotion events and awareness campaigns
* Building strong partnerships with healthcare, mental health and substance misuse services
* Supporting the delivery of a varied programme of wellbeing activities, including evenings and weekends
* Maintaining accurate records, care plans and case management documentation What We're Looking For We're seeking someone who is: * Passionate about health, wellbeing and recovery
