What you will do:

Conduct physical and mental health assessments for individuals accessing the services, ensuring proper referrals for further care when necessary.
Lead assessments for alcohol service users and assist in determining safe detoxification plans.
Support the titration process for opiate users, ensuring optimal dosing and clear communication with team members and community pharmacists.
Provide education, health promotion, and screenings for blood-borne viruses, alongside administering Hepatitis B vaccinations as needed.
Undertake community detoxification for alcohol and opiates, either at the service locations or in the service users' homes.
Contribute to the Naloxone Strategy by training individuals in its use and distributing it to service users.
Offer wound care assessments and basic wound treatments, with referrals to other services as appropriate.
Engage with service users through Integrated Test and Evaluation Plans (ITEP) to enhance recovery planning and treatment engagement.
Maintain and communicate up-to-date knowledge of clinical practices and interventions, attending relevant meetings and training under the guidance of the Sister/Charge Nurse.
Drive the delivery of high-quality care services, constantly reviewing practices to align with clinical guidelines and protocols.
Provide clinical leadership by supporting and coaching staff, ensuring their continued professional development in harm reduction and treatment approaches.
