Job summary
We are looking for a highly motivated Occupational therapist or Physiotherapist to join our multidisciplinary Pro-active Care Hub (PACH) team. This is a new service with an emphasis on proactively helping local people to remain well at home, by ensuring their long term conditions are well managed. The PACH will have a core team of nurses and other health care professionals, who have knowledge and skills in supporting people who have frailty and long term conditions. Main duties of the job
The post holder will lead on developing the therapy provision for PACH, in collaboration with the multidisciplinary team. They will develop, implement and evaluate a high quality specialist service, ensuring that patients receive the highest standard of care, complying with evidence based practice. The post holder will regularly report progress for clinical governance and audit purposes. - In collaboration with MDT colleagues, develop annual plans for the proactive Long term conditions hub, incorporating National Guidelines for service (i.e. NSFs and national guidelines) and ensuring completion of objectives at year-end.
