Practice Nurse

Job summary
We are seeking to recruit an experienced, highly motivated Practice Nurse to join our friendly, innovative team who are committed to delivering high standard of patient care. You will need to have previous experience working in primary care and have a fully rounded set of skills. You will work both independently and as a member of our multidisciplinary team and will be fully supported by clinical and non-clinical staff.

Job responsibilities

• We are looking to recruit someone with experience in practice nursing and chronic disease management desirable.
• Based across 2 sites.
• Equal opportunities employer with approx. 7,500 patients.
• Our clinical system is System One (training available if necessary).
• Salary dependent on qualifications and experience.

Qualifications
Essential

• Registered Nurse with current NMC registration.
• Evidence of continuing professional development.
• Qualified and competent in immunisations and vaccinations.
• Qualified and competent in cervical cytology (smears) or willing to complete the necessary qualification

Desirable

• General Practice Nursing qualification.
• Chronic Disease Management qualification(s).
• Independent Prescriber qualification.
• Mentorship/Supervision qualification.

Experience
Essential

• Experience working as a Registered Nurse.
• Experience managing long-term conditions including:
• Diabetes
• Asthma
• COPD
• Hypertension
• Cardiovascular disease
• Experience undertaking cervical screening or willing to learn
• Experience administering vaccinations and immunisations.
• Experience in venepuncture/phlebotomy.
• Experience carrying out diabetic foot checks or willing to learn
• Experience delivering health promotion and lifestyle advice.
• Experience maintaining accurate clinical records.
