Job summary
The post holder will be an experienced healthcare practitioner with experience in acute and urgent care. Ideally they will have completed an MSc in Advanced Practice, and must be independent prescribing. The post holder must be able to evidence a clinical portfolio and relevant competencies in line with the HEE national framework for advanced clinical practice. Advanced clinical practitioners are expected to work across the four pillars of advanced practice; clinical, leadership, research and education. The post holder will practice as an Advanced Clinical Practitioner/Paramedic across various services, including but not limited to ABC Remote Services (uCATS, Enhanced Access, Additional Primary Care, Smoking Cessation and ADHD Primary Care Connect) The post holder will autonomously undertake assessment and treatment of both acute and chronic, differentiated and undifferentiated, patient presentations. The post holder will determine a clinical diagnosis, treatment plan and appropriate discharge, or refer, as indicated whilst maintaining accurate patient records and ensuring patient safety. They will be a source of clinical expertise to less experienced staff, enhancing skills and knowledge, and work in partnership with a multidisciplinary team. Training will be provided to equip the post holder to manage the prescribing of smoking cessation medication as part of a collaborative service with West Sussex County Council. - To independently undertake specialised clinical assessments, diagnose, treat and discharge, or appropriately refer, patients with a wide range of acute and chronic illnesses.
- To ensure patients are fully involved in the design and implementation of their treatment plans.
- To ensure effective coordination and liaison with multidisciplinary teams across primary and secondary care, statutory and voluntary disciplines/groups to provide a seamless service to patients and their carers and prevent hospital admission where urgent care is required.
- To undertake assessments as part of the ADHD Primary Care Connect service, diagnosing and prescribing medication as appropriate and with the support of the Consultant Psychiatrist
- To prescribe smoking cessation medication in line with service protocols
- To refer appropriately to clinical leads/supervising GP/Consultant Psychiatrist where a second opinion, or supervision, is required.
- To adapt styles of communication appropriately to promote empowerment, self-management, and health education for patients.
- To attend clinical case conferences, MDTs, review and clinical meetings as required.
- To be a source of clinical expertise to less experienced staff within the team.
- To delegate duties/tasks to other members of staff where appropriate.
- Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relatives, and carers.

- To ensure that patient records and documentation are maintained in line with recognised standards and ABC policies and use of the electronic patient records system. This includes
