Job Title: Social Worker – Community Discharge to Assess
Location: Warrington
Rate: £30 per hour

We are seeking a dedicated and compassionate Social Worker to join our Community Discharge to Assess Team (D2A) in Warrington.

Responsibilities:

In this role, you will be responsible for conducting comprehensive Care Act assessments for individuals who have been discharged from hospital into a temporary 24-hour care placement. Your primary objective will be to assess the individual’s care needs, identifying appropriate services, and coordinating the care and support. This is a fast-paced role, where timely and quality assessments are needed. As a vital member of our social work team, you will play a crucial role in supporting individuals to achieve their outcomes. Candidates must have D2A experience to be considered for the role. You will be conducting comprehensive Care Act assessments for individuals discharged from hospital into temporary 24-hour care placements, assessing care needs, identifying appropriate services, and coordinating care and support. This role requires timely and quality assessments to support individuals in achieving their outcomes.

Qualifications and Experience:

• Social Work England registration
• Eligible to work in the UK
• Hold a full UK licence
• Post-qualified experience
• Degree level or equivalent in Social Work
