Job summary
The Sunderland Learning Disability Community Treatment Team is delighted to offer an exciting opportunity for a Band 5 Community Nurse to join our Positive Behaviour Support (PBS) Pathway.

We are a multidisciplinary community team providing specialist support to individuals with learning disabilities and complex needs across Sunderland. We are seeking a motivated and compassionate nurse who is committed to delivering high-quality, person-centred care. The successful candidate will possess strong organisational skills, be able to effectively prioritise their workload, and contribute to maintaining a service that is safe, caring, responsive, and well-led.

The role involves working collaboratively with a wide range of partner agencies, including social care teams, care providers, inpatient services, safeguarding teams, and other key stakeholders. Our team brings together a wealth of expertise and experience, enabling us to effectively support individuals with complex needs while managing associated risks in a proactive and positive manner.

Main duties of the job

• To collaboratively engage in the assessment and management of a caseload of patients who have substantial and complex needs.
• Provide clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public.
• You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision as required.
• Work collaboratively with patients to develop care plans, recovery and wellbeing plans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary.
• To consider carer needs and ensure carer and family are involved in care planning where appropriate.
• To deliver evidenced based clinical interventions.
• The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence.
• Coordinate treatment planning, interventions, clinical reviews and discharge planning.
• Provide clinical supervision to staff as delegated by Pathway Manager and Clinical Leads.
• To provide compassionate care that is based on empathy, kindness, respect and dignity.
• To incorporate a patient centred philosophy into nursing practice and patient centred collaborative care.

Person Specification
Education and Qualification
Essential

• First Level Registration RMN/RNMH/RNLD Qualification
• Experience of working with service users and carers in successful delivery of treatment plans
• Complete preceptorship programme for newly qualified nurses
• Multi-disciplinary team work.
• Proven ability to develop therapeutic partnership with service users and carers.
• Proven ability to be able to communicate interventions to service users.

Desirable

• Further training relevant to the post
• Previous experience in a community setting.
• Experience of involving service users/ carers in evaluation/planning and monitoring of services

Knowledge and Experience
Essential

• Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
• Understanding of the statutory issues which impact on service users.

Skills and Competencies
Essential

• Ability to use IT system
• Comprehensive continuous assessment of skills
• Risk Assessment skills.
• Risk Management and Safety Planning skills.
• Ability to establish a rapport and demonstrate engagement skills with service users
