We have an exciting opportunity for an experienced IBD nurse to join our small and friendly team. The Trainee Advanced Clinical Practitioner (ACP) will undertake a structured apprenticeship programme aligned to the Royal College of Physicians (RCP) Advanced Clinical Practice in Acute Medicine curriculum and the Society for Acute Medicine (SAM) standards. The post holder will be expected to take an active role in the respond, stabilise and make safe element of acute care pathways by delivery of advanced assessment, and the management and treatment of patients over 16 years of age patients with inflammatory bowel disease (IBD) referred to IBD Acute Flare Clinic. This post supports the Trust's strategic ambition to:
The Advanced Practitioner role, within the IBD service, will provide comprehensive healthcare sensitive to the needs of the population, including advanced clinical assessments, history taking, diagnosis, plan and deliver treatment, prescribe and evaluate. Working as part of a multidisciplinary team, the post holder will be responsible for co-ordinating the delivery of urgent health care needs of people with IBD with an exacerbation of their disease within the IBD Acute Flare Clinic at risk of hospitalisation. The post holder will provide specialised in-reach to the Emergency Department to triage, assess and input as an independent practitioner to patients with an exacerbation of their IBD. The focus will be to prevent unnecessary acute hospital admissions and enable patients to remain safely and supported at home and prevent premature admissions or readmissions. The post holder will provide specialist care to inpatients with IBD, assist patients to proactively manage and anticipate their future health needs and develop appropriate plans in conjunction with the Consultants and the IBD service. It is expected that the post holder will provide IBD care in an outpatient setting to both new and already diagnosed patients. The post holder will work towards in developing, delivering and evaluating high quality patient centred interventions in conjunction with the associated clinical teams. About us

- Clinical practice working under supervision: Provide a level of advanced and comprehensive nursing assessment based on a specialist body of knowledge
- Receives referrals for new/suspected and established patients. Has appropriate, up-to-date, clinical education including advanced assessment skills and non-medical prescribing (in line with local guidelines or protocols).
- Uses advanced clinical skills and judgement in managing complex and unstable IBD. Assesses, plans, implements and evaluates care for patients.

- Working at an advanced level as a nurse or other registered allied health professional
- Current or recent experience in IBD
- Significant post registration experience with evidence of autonomous practice
- Experience in managing critically or unwell IBD patients
- Experience in medication counselling of advanced therapy for IBD patients
- Experience in liaising with key stakeholders
- Experience in clerking patients from front-door pathways (ED/GP)
