We are seeking an enthusiastic and motivated Practice Nurse to join our welcoming team and play a key role in delivering outstanding care to our local community. Whether you're an experienced Practice Nurse, or newly qualified, this could be the perfect opportunity for you.

This is an exciting opportunity to work within a supportive and collaborative practice. If you are new to General Practice, we support our new Nurses through a Practice Nurse Preceptorship.

Main duties of the job
As a Practice Nurse, you will provide high-quality nursing care to patients across a broad range of services, to include all duties normally undertaken by an RGN and in addition any duties as appropriate.

We see patients at both sites and have a team that includes GPs, paramedics, nurse practitioners, practice nurses, health care assistants and phlebotomists as well as our practice management and administrative support teams. In addition, we also have access to clinical pharmacists, social prescribers, and a mental health practitioner through our local Primary Care Network.

Job responsibilities
Professional:
  • Practice independently in accordance with NMC code of conduct and with reference to practice guidelines, appropriately referring to GPs
  • Maintain accurate documentation of care and utilisation of IT
  • Ensure continuation of own personal and professional development, facilitated by utilisation of clinical supervision networks.
  • Offer a holistic approach to travel health by providing comprehensive advice to patients prior to travel including; vaccinations and medicines, safe sex, food hygiene, sun protection. Refer to Travel Clinic if complex
  • Offer advice about childhood and adult vaccinations and ensure vaccines are administered under patient group directions
  • Running chronic disease clinics and health promotion programmes by providing a holistic assessment and lifestyle advice on diet, smoking, alcohol intake and exercise. Facilitates change
  • Assessing problems presented opportunistically by patients, dealing with minor injuries.
  • Able to document consultations according to NMC guidelines
  • Ensures awareness of statutory and local clinical protection procedures, including systems of referral. Ability to recognise signs and symptoms of child abuse
  • Perform a holistic assessment of patients attending for ear care including irrigation
  • Perform a holistic assessment of patients attending for cervical cytology smear tests
  • Perform venepuncture according to local guidelines
  • Provide a holistic approach to wound management and implement wound care in line with current evidence based guidelines
  • Suture removal
  • Take ECGs
  • Assist in the provision of minor surgery
  • Ability to obtain and document informed consent (either verbal or written)
  • Ensure infection control guidelines are maintained
  • Ability to monitor and manage maintenance of stock and equipment to include refrigeration, sterilizer and emergency equipment

Managerial:
  • Contribute to the assessment of service needs
  • Assist patients to identify their health needs
  • Contribute a nursing perspective to the practice development plan
  • Manage and organise individual patient consultations

Person Specification
Skills and Knowledge
Essential
  • Knowledge of patient group directions and associated policy
  • Clinical skills chronic disease management, cervical cytology, immunisation and vaccination
  • Change-management skills and ability to support patients to change lifestyle
  • Communication skills, both written and verbal
  • Ability to communicate difficult messages to patients and families
  • IT skills, competent with Microsoft Office 365, email, and use of internet as a resource
Desirable
  • Understanding and knowledge of healthcare provision in GP practices, QOF and enhanced services
  • Awareness of local and national health policy
  • Knowledge of public health issues in the area
  • Knowledge of clinical governance issues in primary care
  • Audit skills
  • Knowledge of Primary Care IT systems i.e. SystmOne
Experience
Essential
  • Post registration/working towards experience in a primary care setting
  • Long-term condition management
  • Experience working autonomously and as part of a team
Qualifications
Essential
  • Educated to Degree level (relevant Nursing/ General Practice Nurse qualification)
Desirable
  • Independent/supplementary nurse prescribing qualification
  • Minor injuries qualification
  • Chronic Disease management qualification
