Main duties of the job
The Role As a key member of our clinical team, you will help us keep quality at the heart of everything we do. Your responsibilities will include:

• Conducting holistic patient assessments and creating tailored care plans
• Delivering immunisations and health screenings
• Running chronic disease clinics (e.g., diabetes, asthma)
• Providing family planning advice and performing cervical screening
• Supporting minor surgical procedures
• Promoting health education and disease prevention
• Maintaining accurate patient records and documentation
• Contributing to audits and quality improvement initiatives

About us
We are a practice of almost 7000 patients across two sites. We have two GP partners and host registrar doctors. We have a nursing team, admin team and full complement of additional roles such as Clinical Pharmacist, Mental Health practitioner and First contact Physiotherapist.

Job responsibilities
Provide care and management for patients with long-term conditions (e.g. asthma, COPD, diabetes, heart failure, hypertension). Perform health screenings, cervical smears, vaccinations (including childhood, travel, and campaign programmes). Support chronic disease management in line with the Quality and Outcomes Framework (QOF) and evidence-based guidelines. Promote health education, lifestyle advice, and preventative care. Maintain infection control standards and complete audits as required. Contribute to the development of integrated, multidisciplinary working across the practice. Supervise and support HCAs and other team members where needed. Keep accurate and up-to-date patient records in line with policies and NMC standards. Participate in annual appraisals, training, and ongoing professional development (CPD).

Person Specification
Qualifications - Essential

• Nursing degree
• Membership of a professional body NMC (Nursing Midwifery Council)

Experience - Essential

• Conducting patient assessments and developing care plans.
• Providing immunisations, wound care, and health screenings.
• Managing chronic disease clinics, including diabetes and asthma.
• Offering family planning advice and conducting cervical smear tests.
• Assisting with minor surgical procedures.
• Providing patient education on health promotion and disease prevention.
• Ensuring accurate documentation and maintaining patient records.
• Participating in clinical audits and quality improvement initiatives

Experience - Desirable

• Knowledge of health promotion strategies
• Aware of accountability of own role
• Knowledge of needs of patients with long-term conditions and chronic disease
• Knowledge of patient group directions and associated policy
• Clinical skills cervical cytology, immunisation and vaccination,
• Change-management skills and ability to support patients to change lifestyle
• Communication skills, both written and verbal including communicating difficult messages to patients and families
• Negotiation and conflict management skills
• IT skills
• Ability to work on own initiative
