Job summary
Do you want to join a team who are striving to improve care and experience for patients with rare cancer? An exciting opportunity has arisen for a patient focused, driven cancer nurse to join the South West Peninsula Sarcoma multi- disciplinary team. Working alongside the Sarcoma Nurse Consultant and support worker, the successful candidate will lead the team in providing expert clinical care to patients with Sarcoma Cancer . Excellent communication skills, motivational skills, and the ability to work as part f a multi- disciplinary team are essential. Please note that applications from temporary workers who have been engaged with the Trust for a minimum of 3 months are welcome. Main duties of the job
Working across Plastics, Hepatobiliary and Oncology to assess, plan, implement and evaluate expert nursing care and coordinate and provide support to patients with sarcoma cancer and their carers through their diagnosis and treatment and to ensure those living with and beyond cancer treatment have access to appropriate support and information. To undertake/facilitate holistic assessment and care plan, end of treatment summaries, implement and evaluate expert nursing interventions for cancer patients. To provide specialist nursing advice and psychological support in a consultancy role to patients, carers and other healthcare professionals. To develop & undertake nurse led clinics To act as an expert resource and facilitator to educate and train other Healthcare professionals as required. To lead and develop the service according to need, undertake audit and research projects and develop policies and procedures as required. To work as an integrated member of the plastics/hepatobiliary and oncology specialists' team. About us

- Works autonomously, in accordance with NMC guidelines, to manage his/her caseload of patients whilst working as part of the cancer multidisciplinary team.
- Develop and undertake nurse led initiatives and clinics with support of consultants and with other nursing leads within Plastic Surgery.
- Act as key worker for cancer patients. Undertake holistic needs assessment in line with national and local guidance.
- Communicates highly sensitive and emotive condition related information to patients, relatives e.g. terminal illness, bereavement etc.
- Participates in complex ethical decision making process.
- To act as the patients advocate on a frequent basis and provide expert advice on treatment options to enable patients to make informed choices
- Provides continuity of care throughout the patients journey and liaises across organisations
- Develops specialised programmes of care and provides highly specialised advice and written and oral information to patients, relatives
- To act as a key communication resource for the multidisciplinary team. Provides continuity of care throughout the patient pathway across specialised areas, including dermatology, plastic surgery, oncology, and palliative care.
