Are you passionate about improving the lives of older adults living with dementia and mental health needs? We are looking for a compassionate, motivated and skilled practitioner to join our Older People's Mental Health Service, working as part of the ICT in Farnham. In this rewarding role, you will provide high-quality, person-centred mental health and cognitive assessments, brief interventions, care coordination, and signposting for adults with dementia and associated mental health needs who are housebound, living in care homes, or supported within primary care. You will help ensure people receive timely support, reducing the risk of deterioration and preventing individuals from falling between services. Working collaboratively with GPs, community teams, social care, voluntary organisations and specialist mental health services, you will use your expertise to support patients, carers and families, contribute to personalised care planning and risk management, and facilitate appropriate onward referrals where needed. The post holder will: undertake holistic mental health and cognitive assessments support post-diagnostic interventions and reviews provide recommendations to primary care colleagues support care planning and risk management facilitate onward referral where required strengthen integration between primary care, community services, memory services, voluntary sector and secondary mental health services. The role will also contribute to education, consultation and professional support within the wider multidisciplinary team to improve understanding of dementia and mental health needs within Farnham community settings.
