* Assess, order, and review specialist equipment for patient needs.
* Coordinate complex discharge pathways from acute and community settings.
* Manage a specialist caseload and coordinate patient flow efficiently.
* Lead case conferences to support safe and timely discharge.
* Provide specialist clinical advice to patients, carers, and professionals.
* Support admission avoidance and facilitate earlier discharge.
* Make recommendations regarding healthcare packages and onward referrals.
* Liaise with multidisciplinary teams, community services, and care providers.
* Maintain accurate clinical records and documentation.
* Support service development, audits, and quality improvement initiatives.
* Mentor, supervise, and appraise junior staff members.
* Deliver training on discharge planning and Intermediate Care pathways.
