Children's Community Nurse (Complex Care) Hounslow

An outstanding job opportunity has arisen for a Paediatric Community Nurse to work for an Social Care provider in London.

In this specialised role, you will provide coordinated nursing care to children in the community, ensuring they receive the essential support they need. You will offer a wide range of clinical nursing services, including wound management, medication administration, end-of-life care, symptom control, palliative care, catheter care, and phlebotomy, all tailored to meet the unique needs of your patients.

As part of one of the UK’s social care providers, you will be delivering high-quality care and support within the community. You will be an integral member of a dedicated nursing team, committed to delivering exceptional care to housebound patients.

As community nursing evolves at a fast pace, the ability to embrace change and adapt to new practices is essential.

To be considered for this position, you must be qualified as a Nurse with a current active NMC Pin.

The following skills and experience would be preferred and beneficial for the role:

• A minimum of 12 months experience
• Ensure the delivery of quality care by continually assessing our service user needs and wishes
• Develop, review and update care plans to meet our clients changing physical, social and psychological needs
• Comply with all legal, regulatory and best practice guidelines Ensure medicines are appropriately received, stored and administered to our service users
• Range of extended clinical skills & demonstrate clinical knowledge.
• Male & Female catheterisation
• Venepuncture and immunisation
• Able to communicate sensitive information to patients, relatives and carers
• Able to assess, plan, implement & evaluate packages of care
• Able to develop effective interpersonal
• Skills/relationships across the health & social care setting
• Good verbal & written skills

Please note you will be expected to cover days, nights and weekend shifts as part of a weekly rota.
