Salary: Band 7/8 £28,142 to £33,699,
Permanent, 37 hours per week x 12 posts
Base is to be within the Bradford district.

About the Role
Our service is continuing to grow and are increasing our offer of support for to families where children are at risk of being cared for outside of their family and are returning home after time living in care.
As a Strengthening Families Outreach Practitioner, you will work children, young people, and their families alongside the allocated Social Worker, delivering intensive, bespoke support to enable families to remain living together safely wherever possible. You will provide practical, relationship-based interventions within the family home and wider community, tailoring support to meet individual needs and using evidence-based approaches to promote positive and sustainable change and actively participate in multi-agency forums, including child protection conferences, core groups, and risk management panels, advocating for families while ensuring that the voice of the child remains central to all interventions.
Working collaboratively with multi-agency partners, you will contribute to coordinated plans that prioritise the safety, stability, and wellbeing of children. You will maintain accurate and reflective records of each family’s journey, evidencing progress and outcomes.

You’ll also
Work directly with children and families in their own homes and communities, identifying the appropriate interventions/tools to deliver positive outcomes. This includes working early mornings, evenings and some weekends when the need arises.
Contribute to holistic assessments of need and devise and implement appropriate plans for babies, children, young people, and families.
Key work families with a range of complex needs such as drugs, alcohol, mental health, and domestic abuse to support and motivate them to make changes to improve outcomes for children and young people.
Review our support package on a regular basis to track and show progress, including children, young people, families, and partner agencies as appropriate, working in partnership to achieve the best possible outcomes for children, young people and families.
Maintain accurate up to date and appropriate records using electronic systems and equipment in line with GDPR regulations.
Write reports and attend meetings, reviews court etc. as required, representing the department.
Ensure the appropriate participation of children and young people in their own plans.
To be successful in this role you will need:
- Minimum of 2 years continuous & recent experience working directly with children, young people and parents as part of a multi-disciplinary team to plan and support change in families.
- A relevant Level 3 professional qualification related to working with children, young people and parents and evidence of on-going training and CDP.
- To manage you own workload and work in an unsupported environment with families in their own home.
- Applies anti discriminatory practice and approaches in their day-to-day interactions with colleagues, children and their families to challenge bias, prejudice and intolerance.
- Be able to work evenings, weekends and Bank Holidays as required by the needs of the service.
- The post holder is required to have a useable car available at all times or as advised by the line manager. The post is designated Casual Class 1 Car User status for the better performance of the duties for which the HMRC rate is payable.
