- Leading and developing clinical pharmacy services to meet the needs of the emergency & integrated care (EIC) division.
- Providing senior leadership support to the specialist clinical pharmacists and across the EIC team.
- Implementing trust wide initiatives to improve the use, management and safety of medicines across the trust
- Deputizing for the Associate Chief Pharmacist, Clinical Services and assisting in trust wide medicines optimisation work streams
- Lead and be a role model for clinical pharmacy services to the clinical areas managed by Emergency and Integrated Care (EIC) Division, including Respiratory, Endocrinology, Gastroenterology, Cardiology, Care of the Elderly, Rheumatology, Emergency Medicine, ambulatory emergency care and acute medical unit patients.
- Lead, provide and develop clinical pharmacy services to meet the needs of the EIC Division in accordance with objectives set by the Associate Chief Pharmacist, Clinical Services in order to meet Trust objectives and the Clinical Governance Agenda
- Lead by example in an agreed specialism e.g. cardiology or care of elderly ensuring a consistently high quality clinical pharmacy service is delivered to all areas of the EIC Division in collaboration with the multidisciplinary team.
- Manage the clinical service provided by a team of pharmacists and technicians within the EIC Division.

Leadership
- Lead in the development and delivery of pharmaceutical issues to the EIC Division.

- Ensure that issues related to pharmaceutical care are appropriately represented within the EIC Division.
- Work collaboratively with pharmacy and medical staff, the multidisciplinary team and management in the national agenda for pharmacy services in accordance with the agreed objectives.
- Ensure that issues around prescribing and medication use are managed within the development of Electronic Patient Records and prescribing system
- Act as the Deputy for the Associate Chief Pharmacist, Clinical Services when appropriate.
- Provide clinical pharmacy services for patients admitted to medical wards and Emergency Department in accordance with the priorities set by the post holder and Associate Chief Pharmacist, Clinical Services.
- Identify and prioritise clinical pharmacy services to the EIC Division.
- Ensure all clinical areas within the division are covered to provide the most appropriate service within the resources available.
- Ensure national and local agendas are delivered.
- Establish processes for the setting of standards of care; identify and manage changes that need to occur to achieve these and then manage the risks involved.
- Manage and appraise the designated trainee pharmacist, diploma and MSc students and specialist pharmacists.
- Be responsible for recruitment and retention of staff for the Medicine and A&E clinical pharmacy team.
- Advise and support on the development and implementation of shared-care guidelines in order to promote seamless care for patients.
- Effectively monitor and report on the divisions drugs budget and provide feedback to service managers, clinicians and accountants at division level.
- Identify changes in prescribing practices that may impact on the drugs budget, including new prescribing practices (horizon scanning).
- Provide advice on the cost effective use of medicines within the division including the implementation of prescribing guidelines in this area.
- Oversee the provision of medicines to patients at home using home care delivery companies.
- Demonstrate professional accountability to service users, stakeholders and the profession
- Review drug related incidents that occur within the EIC Division and implement actions to reduce re-occurrence. This involves membership of the Trusts Drug Incident Committee.
- Assume the responsibilities described in the Trust Major Incident Plan and to implement appropriate actions in the event of such an incident.
- Responsible for the investigation of errors and complaints involving the pharmaceutical services delivered to the EIC Division, liaising with the Associate Chief Pharmacist and division risk managers to ensure a satisfactory outcome.
- Deputise for the Associate Chief Pharmacist, Clinical Services when appropriate in her absence, managing the team and dealing with matters.
- Develop the provision of clinical pharmacy services to EIC outpatient clinics and support the accurate and effective screening processes within the outpatient dispensary.
- Liaise closely with the Senior Technicians to roll out medicines management services to the EIC Division.
- Liaise closely with the Senior Technicians regarding the delivery and development of technician-led medicines management service to the EIC Division.
- Represent Pharmacy / EIC Division on Trust committees or groups where appropriate.
- Provide advice on the implementation and maintenance of Patient Group Directions used in the EIC Division.
- Monitor and evaluate clinical pharmacy services for patients admitted to the EIC Division.
- Evaluate the contributions of the EIC Division pharmacists to the division by the way of twice yearly intervention monitoring, pharmacist transcribing and the evaluation of on-call data.
- Ensure departmental standards are met.
- Supervise and undertake clinical audit as part of the multi-disciplinary audit process
- Undertake assessments / evaluations of services, patient care and individuals as required.
- Act as a clinical role model for the EIC Division pharmacists and demonstrate the ability to provide safe, clinically effective and cost efficient use of drugs in these patients.
- Provide highly specialist pharmaceutical care into the care of cardiology patients (or other speciality of Medicine).
- Participate in the post-take ward rounds to integrate into the multidisciplinary team and raise the pharmacy profile via contributions at consultant level.
- Act as an independent prescriber within the agreed scope of practice as appropriate.

- Maintain records of drug use by specialist area in order to resolve queries from finance.
- Support all clinical staff in the use of Medicines outside of the product licence (off label) and unlicensed Medicines in the division in order to ensure that all medication use is of a high quality and safe.
- Make appropriate referrals where necessary; ensure clear and effective communication of patients pharmaceutical needs to other pharmacists and health care professionals both within the trust and in other health care environments.
- Be responsible and accountable for providing a clinical pharmacy service for patients admitted to the EIC Division.
- Demonstrate a critical approach towards pharmacy practice in the EIC Division.
- Integrate research evidence into practice.
- Undertake own practice research and disseminate findings.
- Guide and support others undertaking research.

- Identify training needs of staff and develop strategies to meet them in order to undertake the roles required within the EIC Division.
- Identify own training needs and maintain a portfolio of practice.
- Tutor trainee pharmacists in their project work.
- Be a mentor for a junior pharmacist, undertaking formal appraisal and support to meet their objectives and career aspirations.
- Provide teaching on the Certificate in Clinical Pharmacy Training Course.
- Contribute to the Clinical Pharmacists teaching programme by presenting at least one session per annum.
- Develop strategies to meet the training needs of staff on rotation in Medicine.
- Provide appropriate education and training for medical and nursing staff.
- Provide the Pharmacy induction for new junior doctors on a 6 monthly basis.
- Be a ward tutor for a junior pharmacists in training which includes regular (weekly/fortnightly) informal ward accompanied visits and two formal visits in their fixed rotation through the Medicine specialties.
- Act as a GPhC trainee pharmacist Practice Supervisor or Educational Supervisor (depending on experience) as appropriate.
- Provide teaching and support as preceptor for undergraduate pharmacists and doctors who are assigned to the pharmacy department for work experience and/or project work as required
Support the Chief Pharmacist in:
- Provision of drug expenditure information required in order for Finance to recover costs for high cost drugs.
- Act as a resource to the Finance Department in dealing with complex and contentious issues around the use of high cost drugs.
- Participate in other areas of clinical pharmacy practice in order to ensure a broad base of pharmaceutical knowledge
- Participate in oncall, weekend/bank holiday and shift working as well as 7 day working when required on a rotational basis.
- Liaise with other clinical pharmacists to ensure efficient transfer of drug related care.
- Be aware of, and apply, relevant legislation such as the H & SAWA, COSHH, Medicines Act, GMP etc.
- At all times practice in accordance with the Code of Ethics of the Pharmaceutical Society of Great Britain.
- Follow legal, ethical, professional and employers codes of conduct.
- Any other duties as reasonably required by the Chief Pharmacist.
- To provide dispensary cover on a rotational basis.
Education and qualifications

- Vocational Master's Degree in Pharmacy or equivalent qualification
- Registered as a practicing pharmacist with the General Pharmaceutical Council
