Job Summary
The CSH Specialist Heart Failure team are looking for an enthusiastic Band 6 Heart Failure Nurse to join their team which provides services across the community of NW Surrey. These services include the undertaking of patient home visits and seeing patients within Outpatient settings across the locality.

Main Duties of the Job
The Community Heart Failure Service delivers a nurse led service for heart failure patients who reside within NW Surrey.

Job Responsibilities

• To act as a resource for heart failure disease management in primary and secondary care
• To deliver autonomous evidence-based patient care, achievable through direct patient contact
• To be able to carry out a complex clinical assessment and interpret findings.
• To work in conjunction with the multidisciplinary team across both primary and secondary care
• To be responsible for assessing patients cardiovascular needs referring them to other health care professionals as appropriate
• Provide autonomous advice and treatment recommendations with regards to patients specific heart failure and education needs in order to improve health outcomes.
• Promote health through empowering patients, offering appropriate health education, information and advice.
• To make referrals directly in accordance with the skills/competencies of the post holder to enable a smooth discharge
• To keep accurate, contemporaneous electronic and written documentation, care plans and reports
• To involve, support and educate patients and, as appropriate, their family and carers
• To treat all patients as individuals, respecting their privacy and dignity at all times
• To work in partnership with all other professionals and agencies involved in the care of the patient, ensuring that all care is delivered along an agreed patient pathway
• To promote health and wellbeing to the patient and their family or carers
• To have an awareness of current national standards and guidelines relevant to Heart Failure and improve practice and health outcomes where appropriate
• To maintain, develop and monitor standards of evidence based nursing care
• To maintain own professional standards and clinical integrity in line with NMC guidelines
• Promote effective communication and relationships within the team
• Partakes in clinical supervision for own development and maintains a record of own personal development
• Actively participates in clinical team meetings
• Undergo all mandatory training in line with role requirements
• Contributes to team audit as required
• Works with colleagues in the team on the development of current and new services
• To develop and maintain effective communication and liaison systems across the whole local health economy
• To maintain data entry requirements and other records
• To maintain clear and comprehensive patient records as per Central Surrey Health policy
• To be aware of and act upon when necessary, procedures that are in place to protect vulnerable individuals
• To be aware of and work within all local policies and guidelines
• To undertake any other such duties which may be required from time to time as are consistent with the post
• Co-owners are employed to work within Central Surrey Health localities and may be reasonably requested to move base temporarily or on a more permanent basis to meet service needs

Person Specification
Experience

• Minimum of 2 years relevant post registration experience
• Understanding of the issues currently affecting the NHS
• Understanding of risk management
• Knowledge of the clinical governance agenda
• Research and clinical audit experience
• Mentorship or teaching experience
• Experience of working in the community
• Experience and evidence of ability to work within a multidisciplinary team
