Our Health Visiting teams are seeking a Registered Nurse to support the delivery of the Healthy Child Programme 0 - 5 years to the children, young people and families of Herefordshire. We currently have two part time posts available based within the HMG Hereford City team (22.5 hours) and within the WBC Hereford City Team (22.5 hours) fixed term for 12 months. You will be based in a locality, and will work as a key member of the Health Visiting teams in that area. You will gain experience in working in the community, in partnership with children, young people, families, primary care, early years settings and other partner agencies. Health Visiting has lots of development opportunities and there may be opportunity where available in the future to undertake the Specialist Community and Public Health Nursing training to be a fully qualified Health Visitor. You must be able to drive and have access toa vehicle for work. Main duties of the job

- Participate in health education programmes and health promotion activities for the 0-5 service.
- Manage own workload as part of the team under supervision of the Health Visitor.
- Actively participate in the monitoring of children's health needs including undertaking the Child Health Review assessment for Children Looked After, as required by the Local Authority to assess the health and well-being of children living in foster care placements.
- To provide advice and information to parents who are new to the area regarding the Health Visiting offer and sign post to partner agencies as required. Also, to carry out baseline health assessments to include weight and height.
- Review the A+E attendances that come into the service and follow up with parents/carers any A+E attendances and give advice as per the Health Visiting standard. Refer any children that are enhanced and attended A+E to the named Health Visitor.
- Provide information to parents/carers on the immunisations Programme and to liaise with GP surgeries to identify children who have not yet received their immunisations to help and support families with the uptake of their children's immunisations as per the schedule.
- Work closely with the relevant professionals in searching for and identifying health needs within the practice population and support the development of services in response to the identified needs.

- Must hold the qualification of RGN, RSCN or RM and be registered with the Nursing and Midwifery Council.

- Possess basic IT skills e.g. able to use Microsoft Word, Excel etc.
- Experience of managing own workload and ability to prioritise work delegated.
- Must have a knowledge of the public health role within Health Visiting
- Able to demonstrate confidentiality in all aspects of their work
- Able to work as part of a team and demonstrate the ability to work in partnership with children, young people and their families as well as professionals
- Previous community experience
- Knowledge of clinical supervision
