Clinical Pharmacist

Job summary
We are seeking an enthusiastic Clinical Pharmacist who is keen to develop their skills in primary care as part of our pharmacy team. This role offers the opportunity to work closely with Senior pharmacists, GPs and pharmacy technicians while developing expertise in medicines optimisation, long-term condition management and prescribing safety.

As an accredited learning environment with over 5 years experience in developing allied health professionals, we are committed to upskilling our workforce. We offer regular clinical supervision, mentorship and protected learning opportunities. You will be actively supported to develop specialist clinical interests and gain experience across a broad range of clinical areas, including: cardiovascular disease, diabetes, respiratory disease, frailty and medicines optimisation. With suitable experience, you will be supported to work towards an independent prescribing qualification.

Main duties of the job
As a Clinical Pharmacist, you will work with the pharmacy team to support medicines optimisation across the PCN. You will undertake structured medication reviews, support patients with long-term conditions, participate in prescribing safety initiatives and provide expert medicines advice to patients and colleagues. You will work closely with Senior Clinical Pharmacists to develop skills in long-term condition management, quality improvement and clinical leadership. You will also have the support of pharmacy technicians and a dedicated pharmacy administrator. If you have not already completed the CPPE PCEP course, you will be enrolled onto this shortly after joining us.

Key responsibilities include
  • Undertake medication reviews and structured medication reviews.
  • Support the management of patients with hypertension, hyperlipidaemia and other long-term conditions.
  • Support the achievement of QOF and prescribing quality scheme (PQS) targets.
  • Participate in audits and quality improvement, including patient safety alerts.
  • Participate in pharmacist-led duty clinics, responding to on-the-day medication queries.
  • Review high-risk medicines and monitoring requirements
  • Manage complex medication reconciliation post hospital discharge.
  • Provide medicines advice to patients and healthcare professionals.
  • Work closely with senior clinical pharmacists to develop specialist clinical knowledge.

Person Specification
Qualifications
Essential
  • GPhC registered pharmacist
  • Understanding of medicines optimisation and prescribing safety principles.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and within a multidisciplinary team.
  • Commitment to professional development.
Desirable
  • Experience working in primary care
  • Completion or undertaking the CPPE PCEP course.
  • Experience conducting medication reviews
  • Postgraduate Diploma in clinical pharmacy.
  • Experience using EMIS Web and Docman.
  • Experience undertaking clinical audits and quality improvement projects.
  • Experience supporting patients with long term conditions.
  • Interest in developing a specialist clinical area.
