The Sunderland Learning Disability Community Treatment Team is delighted to offer an exciting opportunity for a Band 5 Community Nurse to join our Positive Behaviour Support (PBS) Pathway. We are a multidisciplinary community team providing specialist support to individuals with learning disabilities and complex needs across Sunderland. We are seeking a motivated and compassionate nurse who is committed to delivering high-quality, person-centred care. The successful candidate will possess strong organisational skills, be able to effectively prioritise their workload, and contribute to maintaining a service that is safe, caring, responsive, and well-led. The role involves working collaboratively with a wide range of partner agencies, including social care teams, care providers, inpatient services, safeguarding teams, and other key stakeholders. Our team brings together a wealth of expertise and experience, enabling us to effectively support individuals with complex needs while managing associated risks in a proactive and positive manner. To collaboratively engage in the assessment and management of a caseload of patients who have substantial and complex needs. Provide clinical advice to patients, families, carers, staff, statutory agencies, third sector partner agencies and the public. You will be expected to provide high quality assessment and care, alongside offering the wider team support, advice and supervision as required. Work collaboratively with patients to develop care plans, recovery and wellbeing plans. Ensuring patients have access to regular reviews to monitor progress and adapt care plans as necessary. To consider carer needs and ensure carer and family are involved in care planning where appropriate. To deliver evidenced based clinical interventions. The post holder will work as a member of the Multi-disciplinary Team ensuring that high quality individualised care is delivered which ensures safety and a commitment to promoting recovery and wellbeing and maximising independence. Coordinate treatment planning, interventions, clinical reviews and discharge planning. Provide clinical supervision to staff as delegated by Pathway Manager and Clinical Leads. To provide compassionate care that is based on empathy, kindness, respect and dignity. To incorporate a patient centred philosophy into nursing practice and patient centred collaborative care. - First Level Registration RMN/RNMH/RNLD Qualification
- Experience of working with service users and carers in successful delivery of treatment plans
- Complete preceptorship programme for newly qualified nurses
- Multi-disciplinary team work.
- Proven ability to develop therapeutic partnership with service users and carers.
- Proven ability to be able to communicate interventions to service users.
- Further training relevant to the post
- Previous experience in a community setting.
- Experience of involving service users/ carers in evaluation/planning and monitoring of services

- Knowledge of relevant contemporary professional theories of mental health care and evidence based interventions.
- Understanding of the statutory issues which impact on service users.
