What you will do:

* Deliver high-quality mental health services that support recovery, wellbeing, and positive outcomes for service users.

* Provide clinical and therapeutic interventions within community settings, including service users' homes where appropriate.

* Act as Care Coordinator or Lead Professional where required, ensuring coordinated and effective care delivery.

* Work collaboratively with multidisciplinary colleagues and local authority services to provide integrated support.

* Support service users to access community resources and promote social inclusion and independence.

* Build positive relationships with service users, carers, families, and partner agencies.

* Monitor and review care plans, adjusting interventions to meet changing needs and circumstances.

* Maintain accurate, timely, and professional clinical records in line with trust policies and NMC standards.

* Provide leadership, supervision, and support to colleagues and contribute to team development.

* Participate in quality improvement initiatives and service development projects.
