- Deliver a high standard of patient care using high level autonomous clinical skills and a broad and in-depth theoretical knowledge base
- Deal with presenting patients needs in a primary care setting.Responding effectively to patient need and ensuring patient choice and ease of access to services.
- Promote healthy lifestyles and participate in national immunisation and screening programmes.
- Enable patients to manage their long-term health care needs and to achieve their full potential in terms of independent living.
- Carry out all duties in accordance with the NMC Code of professional conduct and with regard to the various local and national policies, guidelines and protocols.

Qualifications Essential
- Registered General Nurse (Currently registered with the NMC)
- Understanding of own accountability arising from the NMC Code of Professional Conduct
- Understanding of relevant legal issues e.g. data protection and confidentiality
- Understanding of equal opportunity and diversity issues.
- Ability to assess and manage patient risk effectively and safely.
- Excellent interpersonal, verbal and written communication skills.
- Ability to work effectively in a multi-disciplinary team.
- Able to build effective working relationships at all levels.
- IT skills and Time management skills
- Able to work within a clear set of guidelines.
- An understanding, acceptance and adherence to the need for strict confidentiality

- Qualification in cytology and or family planning
- Diploma in one or more chronic disease management
- Assessor/mentor qualification
- Experience of EMIS Clinical system
- Experience in travel/child immunisations
- Experience of working in community/primary care
- Chronic disease management
- An understanding of the structure of primary care within the wider NHS.
- Negotiating skills
- Able to demonstrate innovative practice.
- Ability to undertake audits.
