Practice Nurse

Job summary
Practice Nurse essentially improving patient outcomes by delivering preventative care, early intervention, and ongoing support for chronic diseases. To help reduce hospital admissions by ensuring patients receive appropriate monitoring and education within the community. Through their clinical knowledge, communication skills, and compassionate approach, Practice Nurse contribute significantly to the efficient running of GP practices and the overall health of the population.

Main duties of the job

• Deliver a high standard of patient care using high level autonomous clinical skills and a broad and in-depth theoretical knowledge base
• Deal with presenting patients needs in a primary care setting.Responding effectively to patient need and ensuring patient choice and ease of access to services.
• Promote healthy lifestyles and participate in national immunisation and screening programmes.
• Enable patients to manage their long-term health care needs and to achieve their full potential in terms of independent living.
• Carry out all duties in accordance with the NMC Code of professional conduct and with regard to the various local and national policies, guidelines and protocols.

Job responsibilities
Clinical Responsibilities
Undertake clinical duties with relevant training in some or all of the areas:

• Cervical Cytology
• Immunisations
• Ear Care
• Minor Injuries
• Phlebotomy
• Wound Care
• Travel Health Immunisations and Advice

Actively encourage and facilitate choices for patients by providing health promotion during routine screening procedures e.g. new patient health checks, cervical cytology and long-term conditions management.

Communicate complex information to patients and relatives where needed with a variety of diverse needs, using negotiation and persuasive skills to motivate.

Plan, implement and evaluate clinical packages of care, including physical, psychological and social needs.

Develop packages of care in collaboration with patients and other health and social care professionals.

Qualifications

Essential

• Registered General Nurse (Currently registered with the NMC)
• Understanding of own accountability arising from the NMC Code of Professional Conduct
• Understanding of relevant legal issues e.g. data protection and confidentiality
