- Conduct assessments via phone or video, using your clinical skills to gather evidence and document findings.
- Access online and telephone support from clinical team leads and MDT colleagues.
- Registered Nurse (NMC registration) with at least 12 months of post-qualification experience. Your registration needs to be as an adult nurse (RMN, RNLD, or RGN).
- Strong communication, IT, and analytical skills.
- Clinical knowledge of adult health conditions, including both mental and physical health.
