Main duties of the job
The Role
As a Registered Nurse with us, you will:
  • Deliver high-quality, person-centred palliative and end-of-life care
  • Work collaboratively within a skilled, compassionate MDT
  • Support patients and families with empathy, sensitivity and professionalism
  • Contribute to continuous improvement and uphold the highest standards of safety and quality
  • Act as a role model, supporting colleagues and students
  • Use your clinical expertise to assess, plan and evaluate individualised care
Youll work 12-hour shifts across a 7-day rota, supported by experienced Team Leads and a Clinical Lead.

Job responsibilities
Purpose of the Role
As a key member of our multidisciplinary team, you will deliver compassionate, personcentred and evidencebased palliative care to patients, their families and carers. You will help us achieve our vision of excellence in specialist palliative care, acting as a role model and supporting colleagues to maintain the highest standards of practice.

Key Responsibilities
Being an Accountable Professional
  • Work in full accordance with and maintain all professional registration requirements.
  • Work in full accordance with The NMC Code and maintain all professional registration requirements.
  • Apply relevant legal, ethical and governance frameworks to ensure safe, compliant practice.
  • Use and interpret research to inform and enhance clinical decisionmaking.
  • Communicate confidently and sensitively with patients, families, colleagues and external partners.
  • Build and maintain therapeutic, professional relationships.
  • Maintain accurate, timely and comprehensive clinical records using appropriate digital systems.
  • Engage in reflective practice, supervision and ongoing professional development.
Promoting Health and Preventing Ill Health
  • Use skilled communication and strengthbased approaches to support people in making informed choices about their care.
  • Provide clear, accessible information about illness, treatment options, care planning and endoflife decisions.

Assessing Needs and Planning Care
  • Assess the holistic needs of patients and families at end of life, including preferences for care and treatment.
  • Develop personalised, evidencebased care plans with clear goals.
  • Work collaboratively with patients and families to review, evaluate and adapt care plans.
  • Assess mental capacity and apply relevant legislation when supporting decisionmaking.
  • Identify safeguarding concerns and act promptly to protect vulnerable individuals.
Providing and Evaluating Care
  • Deliver compassionate, personcentred care that prioritises dignity, comfort, privacy and safety.
  • Provide highquality endoflife care, including support for the dying person, their family, and the bereaved.
  • Safely administer and optimise medicines in line with national and local policy.
  • Coordinate safe discharge or transfer between care settings.
  • Monitor and evaluate the quality of care delivered by yourself and others.
Leading and Managing Nursing Care
  • Understand and utilise the skills and scope of all members of the clinical team.
  • Lead and manage the care of a defined group of patients, prioritising, delegating and coordinating effectively.
  • Support the education, supervision and assessment of students and colleagues.
  • Provide constructive feedback, promote reflective practice and contribute to a positive learning environment.
  • Use digital systems responsibly to access, share and record information.
Improving Safety and Quality
  • Apply health and safety legislation to maintain a safe working environment.
  • Recognise risks to care quality, including staffing and skill mix, and escalate concerns appropriately.
  • Participate in audits, quality improvement initiatives and service development.
  • Report and reflect on incidents, near misses and adverse events to support learning and improvement.
  • Demonstrate resilience and support others in managing uncertainty and emotional demands.
Coordinating Care
  • Work collaboratively across sectors and agencies to ensure seamless, personcentred care.
  • Support individuals with complex needs to maintain independence and avoid unnecessary interventions.
  • Advocate for people who are vulnerable or have disabilities, ensuring equitable access to care.
  • Make reasonable adjustments to assessment, planning and delivery of care.
Additional Information
This role requires an Enhanced DBS check.

Duties may evolve in line with service needs and professional development.

Qualifications
Essential
  • Registered nurse at degree level, or relevant experience
Desirable
  • Palliative Care qualification
  • Willing to undertake further qualifications

  • Assessing patient needs and of planning, co-ordinating and evaluating care
Desirable
  • Specialist palliative care experience
  • Experience of nursing terminally ill patients and caring for their families
  • Post registration experience
