Main duties of the job
An excellent opportunity has arisen to join our Integrated Transfer of Care (ITOC) Team - multi-skilled team consisting of nurses, occupational therapists, physiotherapists, and Assistant Practitioners. We also work in alignment with social services and other Locala community services.
The ITOC team work across multiple service previsions and bases in the Kirklees district providing clinical triage within the acute setting, in-reach therapy to a community Recovery Hub facility, and complete frailty and falls assessments within our service users own environments.
Our purpose is to support a safe and timely hospital discharge of people who no longer need acute hospital care, to a place more suitable to the person's needs. Assessments and care provision can then be tailored to support people to regain strengths and skills so that they can live as independently as possible. We also aim to avoid admission for patients in A&E and Frailty departments who require intervention to allow them to be discharged home.
You will be working autonomously as well as part of a multi-disciplinary team, liaising with health and social care colleagues to coordinate services, promote recovery from illness, and enable health, wellbeing and self-care. You will be visiting service users within their own homes using goal orientated focus, to maximise their independence, safety and quality of life.
