Our multidisciplinary workforce includes GPs, Practice Nurses, Healthcare Assistants, Clinical Pharmacists, Physiotherapists, Social Prescribers, Paramedics, and Reception and Administration Teams. We are part of the MWEB Primary Care Network (PCN), working collaboratively with Moulton Surgery, Kingsthorpe Surgery, Denton Village Surgery and Woodview Medical Centre to enhance patient services and improve health outcomes across our local population. The Advanced Nurse Practitioner (ANP) is an autonomous senior clinician responsible for assessing, diagnosing, treating and managing patients presenting with acute, chronic, complex and undifferentiated conditions across all age groups. The role requires advanced clinical assessment, diagnostic reasoning and decision-making skills, enabling the practitioner to deliver safe, effective and evidence-based care within their scope of practice. The post holder will independently manage patient consultations, undertake advanced clinical examinations, initiate and review treatment plans, prescribe medications where qualified, and manage urgent and emergency presentations. The ANP will support health promotion, disease prevention and self-management, whilst also contributing to vaccination and immunisation programmes, women's health services, long-term condition management and opportunistic care. A key aspect of the role is the leadership and development of specialist services, particularly respiratory and diabetes care. The ANP will work collaboratively within the multidisciplinary team to improve patient outcomes, enhance service delivery and ensure the provision of high-quality, patient-centred care. The ANP will provide clinical leadership within the practice, acting as a role model and supporting the development of colleagues through supervision, mentorship, education and shared learning. The post holder will contribute to service development, workforce planning, clinical governance, policy development, audit and quality improvement activities. The role requires excellent communication skills, enabling effective engagement with patients, carers, colleagues and external organisations. The ANP will advocate for patients, promote equality, diversity and inclusion, and ensure care is delivered in a compassionate, accessible and person-centred manner The post holder will maintain accurate clinical records, manage clinical risk, ensure compliance with professional and regulatory standards, and support the practice in delivering safe, effective and responsive services. They will participate in audit, quality assurance, safeguarding, infection prevention and control, and continuous professional development activities. Overall, the ANP will combine advanced clinical practice, leadership, education and service improvement responsibilities to support the ongoing delivery and development of high-quality primary care services. - Provide autonomous, high-quality clinical care to patients of all ages within a busy GP practice

- Deliver evidence-based care using advanced clinical assessment and diagnostic skills
- Lead and develop specialist respiratory and diabetes services
- Manage patients presenting with urgent and emergency healthcare needs
- Promote health improvement, disease prevention and self-management to support positive patient outcomes
- Undertake minor illness management, wound care, immunisations and other nursing duties appropriate to competence and training
- Work collaboratively within a multidisciplinary team to ensure safe, effective and patient-centred care
- Contribute to clinical governance, quality improvement, audits and service development initiatives
- Support the education, supervision and development of colleagues and junior staff
- Maintain accurate clinical records and ensure compliance with professional, legal and organisational standards

- Current NMC Registration as a Registered Nurse.
- Masters degree in advanced clinical Practice or equivalent advanced practice qualification.
- Independent Prescriber Qualification V300.
- Advanced Clinical Assessment qualification.
- Recognised Spirometry qualification with evidence of current competency.
- Recognised Diabetes qualification.
- Recognised Asthma qualification.
- Current Level 3 Safeguarding Adults and Children training.
- Evidence of continuing professional development.
- Experience working in General Practice or Primary Care.
- Experience independently assessing, diagnosing and managing patients presenting with acute and chronic conditions.
- Experience of independent prescribing.
- Experience managing patients with long-term conditions.
- Experience undertaking and interpreting spirometry.
- Experience conducting annual diabetes reviews.
- Experience conducting asthma and COPD reviews.
- Experience working within a multidisciplinary healthcare team.
- Leadership or Management qualification.
- Teaching, mentoring or clinical supervision qualification.
- Additional respiratory qualifications.
- Additional diabetes qualifications.
- Experience leading respiratory services within General Practice.
- Experience leading diabetes services within General Practice.
- Experience supervising staff, students or trainee clinicians.
