Paediatric Diabetes Nurse

Job summary
To support the PDSN and Paedaitric Diabetes service with the care, assessment and prompt treatment of children and young people (CYP) presenting to Bedford Hospital NHS Foundation Trust with Diabetes related conditions. To manage care provision within the hospital and community setting, and promote admission avoidance, timely discharge and ensure that care provision is in line with evidence based guidelines. The main responsibilities of the role will be related to the management of young patients with Diabetes with an emphasis on initiating treatment and maintaining high standards of clinical excellence. This will be achieved by active caseload management skills, self-development and motivation in addition to guiding and supporting ward staff. The post holder will work with the Diabetes Team in collaboration with the current PDSN and Consultant(s) with an interest in Diabetes. The post holder will also work under the supervision and guidance of the PDSN's, the Matron for Paediatrics and Head Nurse for Children's Services, as well acting as an integral part of the multidisciplinary team, within the paediatric diabetes service setting.

Main duties of the job
  • Work with the Paediatric Diabetes MDT to support the planning, delivery, monitoring and evaluation of complex care using enhanced clinical assessments, diagnostics, and interventions.
  • Take part in outpatient diabetes management clinics under the supervision of the PDSN', to assess patients and provide appropriate management plans.
  • Attend the acute ward support the ward teams with the admission of patients with T1D
  • To work within the Team to support the diabetes technologies service, keeping up with new developments and technologies through liaison with company representatives and peers.
  • Appropriately respond to daily exposure to hazardous materials such as blood and body fluids and a working environment in which the work pattern is frequently unpredictable.
  • To engage with pathways of transition to adult services for all young people in the service.
  • Develop, implement and regularly review individual management plans

  • oEvidence of recent relevant Diabetes related learning and acute experience

  • oENB 998/MSPP or equivalent teaching Qualification

  • oRelevant demonstrable experience in caring for acutely unwell diabetic patients
  • oDemonstrable autonomous decision making
