An exciting opportunity has arisen for an enthusiastic and motivated nurse to join the Locala Diabetes Service. We are seeking the skills of a Diabetes Specialist nurse who has the ability to make a difference to the health and wellbeing of individuals referred to the service. The role requires a motivated individual who is interested in developing themselves and others to deliver high quality, effective and innovative diabetes care in the community. You will need to demonstrate knowledge of national and local issues and be committed to partnership working with other providers. As part of a team and working in conjunction with GP's, mainstream nursing, social and secondary care services, you will provide a patient focused, co-ordinated case management approach across primary care. The post holder will participate in and influence efforts across health and social services to shape the service, to support patient choice, improve quality of life, and promote self-management whilst adopting the community care team approach. To be shortlisted you will need to meet the essential criteria in the job description. This role is not applicable for sponsorship and candidates must be able to demonstrate their right to work in the UK. About us
