Physiotherapist - REACT

Job summary
An exciting opportunity has risen for enthusiastic and motivated individuals to join our Reactive Emergency Assessment Community Team (REACT) as a Band 6 Physiotherapist. The REACT team is a multi-disciplinary integrated community service within ESNEFT incorporating, Urgent Community Response service, OPAT (Outpatient Parenteral Antibiotics Therapy), OOHs (Out of Hours) and Virtual Ward pathways patients. This role will be working for our Urgent Community Response Team which has developed pathways with East of England Ambulance Service and the Frailty Virtual Ward, two new and exciting government initiatives.

Our team delivers excellent patient focused care to patients in Ipswich and the East Suffolk community. Could you be a valued key member of our team?

The role will be for up to 37.5 hours per week, including weekdays & weekends.

Staff are expected to work 8am to 8pm shifts across the week.

Main duties of the job

• To be an active member of a multi-disciplinary, integrated community team.
• To manage a defined caseload of complex individuals, using evidence based/person centred principles.
• To assess, plan, implement and evaluate therapy interventions.
• To provide leadership within the therapy team
• To provide clinical supervision to band 5 physiotherapists and supervise band 3 and 4 support staff.
• To effectively triage and schedule all therapy referrals to the service in accordance with Trust's policies and procedures and advise junior colleagues on more complex issues.
• To manage a caseload of patients with a range of multiple and complex health needs.
• To utilise a high level of clinical knowledge and experience to carry out assessments, setting objectives and implementing appropriate treatment, as part of the holistic care plan for individuals and/or groups.

Person Specification
Qualifications
Essential

• Diploma or 1st Level degree in Physiotherapy (or equivalent)
• Current HCPC registration
• Clinical educators course within the first year
• Must hold a full valid driving licence and have access to a vehicle or viable alternative

Desirable

• Independent/supplementary prescribing qualification
• Consultation skills
• Coaching skills
• Additional leadership qualification
• Leg ulcer management qualification
• District Nurse qualification

Knowlege
Essential

• Able to demonstrate the ability to work at relevant level
• Knowledge of additional specialist areas through in-house training & short courses
• In-depth knowledge of working within the NHS
• Able to provide high standards of clinical intervention
• Able to review and reflect on own practice and performance and develop a PDP
• Able to support other staff to review and reflect on their practice and develop a PDP
• Able to support workplace learning for staff and students, and specifically act as a field work educator, and preceptor
• Able to analyse complex professional and ethical issues

Desirable

• Able to demonstrate the ability to work within a community health team
• Able to demonstrate the management of complex casework

Experience
Essential

• Documented evidence of continuing professional development
• Post registration experience in a relevant clinical setting

Desirable

• Evidence of previous community experience
