As a Band 6 CHC Nurse , you will be responsible for undertaking comprehensive assessments, reviews, and decision support tool (DST) processes in line with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care. You will work collaboratively with multidisciplinary teams to ensure the delivery of high-quality, needs-based care packages for patients.

Key Responsibilities for the Band 6 CHC Nurse Job in Banbury
- Carry out Continuing Healthcare assessments, reviews, and eligibility determinations using the DST.
- Facilitate multi-disciplinary team meetings to support CHC decision-making.
- Liaise with patients, families, clinicians, and care providers to ensure comprehensive and accurate assessments.
- Coordinate the development of personalised care plans that reflect individual health and social care needs.
- Monitor and review ongoing packages of care to ensure they remain appropriate and cost-effective.
- Maintain up-to-date and accurate records in line with professional and legal requirements.
- Contribute to the delivery of training and support for colleagues and stakeholders around CHC processes.
- Ensure practice is in line with current legislation, policies, and ethical frameworks, including safeguarding procedures.
Requirements for the Band 6 CHC Nurse Job in Banbury
- Registered Nurse with valid NMC registration.
- Experience working in Continuing Healthcare, complex case management, community nursing, or discharge planning.
- Strong knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
- Excellent assessment, analytical, and report-writing skills.
- Confident in leading and facilitating multidisciplinary team discussions.
- Ability to work independently and manage a caseload effectively.
- Excellent communication and interpersonal skills.
- Good IT skills and experience using electronic patient record systems.
Desirable Experience
- Experience working within NHS Continuing Healthcare teams or clinical commissioning groups (CCGs).
- Familiarity with frameworks for mental capacity, safeguarding, and the Care Act 2014.
- Knowledge of community resources and services relevant to long-term care and rehabilitation.
- Full UK driving licence and access to own transport (due to community-based working).
