Care Co-Ordinator

£27k - £28k per annum

Real Care Agency is actively interested in applicants who can bring relevant transferable management, supervisory or operational experience from social care, community services or other people-focused service environments. Care at home is complex, fast-moving and increasingly important. For more than 30 years, Real Care Agency has supported people to live independently in their own homes and communities. We are now looking for a capable and motivated Care Co-ordinator to join our South Lanarkshire team.

This is much more than a rostering or administrative role. It is a hands-on management position in which you will take day-to-day responsibility for a portfolio of care at home services, bringing together people and outcomes, care planning and review, Self-directed Support, workforce leadership, operational problem solving, quality and service development.

We are looking for someone who can think, organise, communicate and lead, while never losing sight of the person at the centre of the service.

What you will be responsible for

· Develop, review and maintain high-quality, person-led care and support plans and risk assessments.

· Work with individuals and families to understand what matters to them and ensure support promotes independence, self-management, choice and control.

· Undertake meaningful reviews and recognise when needs, circumstances or outcomes have changed.

· Understand and monitor Self-directed Support arrangements, commissioned hours and available resources.

· Identify potential over-delivery, under-delivery or budget/resource variance, analyse the position and make recommendations for Line Manager and/or Operational Manager approval. The role does not carry delegated financial authority for SDS budget changes or material financial commitments.

· Roster and coordinate staff effectively to promote continuity, reliability and appropriate matching of staff to individual needs.

· Lead, supervise and support frontline staff, set clear expectations and address performance or practice concerns appropriately.

· Respond effectively to operational problems, staff absence, changing needs and service disruption.

· Communicate confidently with people receiving support, families, social workers, health professionals, commissioners and other partners.

· Maintain high-quality records and support compliance with organisational, professional and regulatory standards.

· Use service information and digital systems to identify issues and opportunities for improvement.

· Contribute to the development and sustainability of the service through excellent delivery, responsiveness to referrals, strong professional relationships and identification of appropriate opportunities for growth.

· Participate in the on-call systems in place providing emergency coverage and crisis intervention support and cover shifts if all other avenues have been exhausted.

Could your experience transfer into care at home?

We are deliberately looking beyond job titles. Relevant experience might come from care or support services, but it could also come from another environment where you have coordinated people, managed competing demands, supervised staff, solved operational problems or delivered services to customers or communities.

What matters is whether you can demonstrate the capability to succeed in a complex, people-centred management role and the values and willingness to develop your knowledge of care at home.

Requirements

What we are looking for

· Experience of supervising, coordinating or managing people, services or complex operational activity, whether gained within social care or through relevant transferable experience.

· Strong organisational skills and the ability to manage competing priorities.

· Confident written and verbal communication.

· Good analytical and problem-solving ability.

· Sound judgement and confidence making decisions within the scope of your role.

· The ability to remain calm and think clearly when circumstances change.

· Confidence working with people, families, staff and other professionals.

· The ability to lead, support and appropriately challenge staff.

· An understanding of person-centred care, outcomes and independence, or the ability and commitment to develop this knowledge.

· Resource awareness without losing sight of people's rights and outcomes.

· Confidence with digital systems, data and new ways of working.

· Personal accountability, initiative and commitment to continuous improvement.

Professional registration and development

The post holder is required to maintain the appropriate SSSC Supervisor in a Care at Home Service registration and to hold, or work towards, the practice and supervisory qualifications required for that registration. Real Care Agency's principal practice route is the SVQ Social Services and Healthcare at SCQF Level 7, historically referred to as SVQ Level 3, together with an accepted supervisory qualification where required.

Real Care Agency provides organisational induction, supervision and learning and development support in accordance with our policies. The post holder remains personally responsible for maintaining SSSC registration, meeting qualification conditions and continuing professional learning requirements, and maintaining the competence required for safe and effective practice.

Hours and requirements

· 37.5 hours per week, working 5 days over 7.

· Participation in the out-of-hours/on-call service is required.
