We're looking for an experienced and motivated GP with Extended Role to join our innovative Inpatient Unit team, supporting older adults and patients with complex clinical needs across Swindon and North Wiltshire. This is an exciting opportunity for a senior doctor seeking a leadership role within a progressive community healthcare organisation. You'll have the chance to shape services, provide clinical leadership, support fellow medical colleagues, and play a pivotal role in improving patient outcomes across our inpatient rehabilitation services. As GP with Extended Role , you'll provide expert medical care across our community inpatient units, working closely with multidisciplinary teams to support patients requiring rehabilitation, recovery, and ongoing management following acute illness. You'll lead clinical decision-making, contribute to service development, support medical workforce planning, and work alongside the Medical Director to oversee appraisals and job planning for medical colleagues. Based primarily at Swindon Intermediate Care Centre , with occasional sessions at Savernake Community Hospital, you'll help ensure patients receive safe, effective, and person-centred care while supporting timely discharge and preventing unnecessary hospital admissions. - Provide senior medical leadership within community inpatient services.
- Undertake comprehensive assessment, diagnosis and management of patients with complex health needs.
- Lead and support multidisciplinary team (MDT) working, including ward rounds and case discussions.
- Develop and oversee care plans for predominantly older, frail patients with multiple long-term conditions.
- Support advance care planning discussions with patients and families.
- Lead medical appraisals and contribute to workforce development and job planning.
- Work collaboratively with community, primary care, hospital and social care partners.
