Are you a passionate and experienced registered nurse looking to make a meaningful difference to the lives of people who are new to Scotland? As a New Scots Community Nursing Coordinator, you will play a pivotal role in delivering high quality, person-centred and culturally sensitive care to refugees, people seeking asylum and other New Scots across Glasgow and surrounding communities. Working as part of a supportive multidisciplinary team, you will lead and coordinate nursing services that promote health, wellbeing and independence while helping individuals navigate and access healthcare services. Duration, Location, and Working Pattern:
- Greenock Health Centre
- Fixed-term for 12 months
- Five out of seven days a week, 08:00 to 18:00, including weekends and public holidays
- Please note the salary is Pro Rata for part-time hours
Key responsibilities:
- Coordinate and manage a caseload of New Scots patients with complex health and social care needs
- Deliver comprehensive assessments, care planning, implementation and evaluation of evidence-based nursing interventions
- Provide clinical leadership, guidance and support to nursing staff, healthcare support workers and multidisciplinary colleagues
- Work collaboratively with health, social care, third sector and partner agencies to ensure effective care coordination and discharge planning
- Support New Scots to access appropriate healthcare services while promoting health improvement, wellbeing and independence
- Lead on clinical risk assessment, incident reporting, quality improvement activities and the implementation of best practice standards
- Contribute to staff development through mentorship, training, supervision and support for new and existing team members
Knowledge, training, qualifications and/or experience required to do the job:
- Registered Nurse with current NMC 1st Level Registration
- Evidence of continuing professional development and commitment to lifelong learning
- Strong communication and interpersonal skills with the ability to engage effectively with diverse communities and partner organisations
- Demonstrable leadership, organisational and case management skills with the ability to work autonomously
- Experience of supporting New Scots, including refugees, people seeking asylum or individuals granted leave to remain
- Excellent problem-solving skills and the ability to work effectively within a multidisciplinary team environment
- IT competency including Microsoft Office applications, email systems and electronic record keeping systems
