Job Summary
We have a 37.5 hour band 4 Community Nurse Associate post in the Shore Locality District Nursing teams in Poole. You will deliver planned care to patients in the community setting. Will support Individual development and training needs for the role and responsibilities. To work effectively within a large team of mixed skills when caring for complex patients in the community setting. Effective communication is key to this role and being aware of multi disciplinary working. Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of. Please note visa sponsorship for this role is unfortunately not available. Ensure you hold valid right to work in the UK and will not require sponsorship if successful. Main Duties of the Job

- To provide a high quality person centred approach to care delivery within your sphere of practice always considering patients privacy, dignity and safety.
- To deliver planned care to a group of patients working within the standards of proficiency outlined by the Nursing and Midwifery Council (NMC) and in accordance with Trust policies and protocols
- To demonstrate the ability to work effectively as part of a team and provide support/guidance to less experienced staff/learners within the clinical environment as needed
- Manage a defined group of patients/service users under the indirect supervision of a Registered Nurse, using evidence based/client centred principles to undertake a range of delegated tasks independently
- Contribute to the ongoing assessment, planning, implementation, and monitoring of patient/service user care, and escalate to senior registered practitioner as appropriate
- To promote health and prevent ill health in service users, contributing to integrated care
- To take responsibility for planned/defined tasks as require
- To plan and undertake clinical tasks guided by standard operating procedures and protocols, within the limitations of the Registered Nursing Associate role.
- Undertakes delegated clinical skills within their scope of practice having once undertaken relevant training, deemed competent and adhere to local and national guidance.

Knowledge skills and training
- Registered Nursing Associate on the NMC register
- Understand and act in line with NMC professional standards
- Ability to develop effective and appropriate relationships with patients, their families, carers and colleagues.
- Ability to communicate effectively (written, verbal, and nonverbal communication) with patients, relatives/ carers and the multidisciplinary team.
- Understanding of the scope of the role of the Nursing Associate in context of the team and organisation, and how the role may contribute to service development
- Ability to organise and prioritise own delegated workload.
- Experience of working in teams under appropriate supervision as part of a multi-disciplinary team.
- Experience of providing and receiving complex, sensitive information.
- Subject to the provisions of the Equality Act, able to travel using own vehicle on Trust business.
- Demonstrable skills in written and spoken English, adequate to enable the post holder to carry out the role effectively.
