TB Nurse | London North West University Healthcare NHS Trust

Provide specialist nurse led TB services Trust wide to minimise/reduce the impact of TB.

To provide specialist nursing advice to support and facilitate high quality, individualised care to patients/clients and relatives/carers without supervision, in collaboration with the multi-disciplinary team in line with local and national targets.

The post holder will provide specialist TB nursing care and support and deputise for their line manager in the management of their clinical speciality. This will involve dealing with complex situations and having to take appropriate action in a variety of settings.

To be flexible in the approach to the clinical area and to provide cover for any area specified acting as professional role model providing direct and indirect clinical care, teaching and expertise.

The post holder will be expected to work across all 3 sites as workload demands; Northwick Park, Ealing and Central Middlesex Hospitals.

The TB team provides specialist advice, leadership and clinical service in relation to TB. The goals of the service are to identify and reduce the risk of people being newly infected with TB and provide high quality treatment and care for all people with TB. The TB team will achieve these goals by making all relevant groups of people aware of the facts of TB, strengthening more active case finding amongst high risk groups including contract tracing and ensuring everyone expected of having the disease has rapid access to TB services. These services are needed by a range of groups with differing needs and are delivered in a variety of settings including community settings, clinics, patients homes and in the acute sector. This is a flexible role to provide care to individuals, families and communities in response to changing local need.

The TB specialist nurse will act as a positive role model for all nurses through demonstration of expert clinical practice in the area of TB. The post holder will provide specialist knowledge and advice to all healthcare professionals, patients and carers.

• Ealing Hospital: a busy district general hospital providing a range of clinical services, as well as 24/7 emergency department and urgent care centre, and specialist care at Meadow House Hospice
• Central Middlesex Hospital: our planned care site, hosting a range of surgical and outpatient services and collocated with an urgent care centre.

Provision of specialist, high quality nursing care and support for patients and their carers.
· Act as a resource for patients and the clinical team by the provision of comprehensive advice, information, teaching and training within LNWUHT.
· Contribute to the development of the service locally.
· Case manage patients with latent TB and a small cohort of active TB cases.
· Identify and keep a record of all cases of tuberculosis, in collaboration with the Health Protection Agency and the NTBS notification systems.
· Contribute to Cohort Review.
· Act as the nurse on call and triage TB referrals.
· Establish links with community groups, general practices, voluntary organisations and other community workers to ensure TB screening is equitable and culturally sensitive.
· Liaise closely with UK HSA and Find and Treat.
· Carry out and interpret the results of specific tests relating to TB and HIV; e.g. Tuberculin skin test, IGRA, Pre/Post HIV test discussion, Viral hepatitis, blood tests, Ishihara test, visual acuity and BCG according to agreed protocols.
· Telephone advice to GPs related to referrals and patient management. Telephone advice to patients and the public related to TB exposure, symptoms, side effects and results.
· Liaise with the UK HSA with regard to the epidemiology of tuberculosis and the management of specific cases.
· Provide nurse-led clinics and domiciliary visits to adults and children including those co-infected with HIV.
· Investigate cases/outbreaks of TB – risk assessment, contact identification, tracing and screening and coordinate mass screening in conjunction with the UK HSA under the close direction of a Band 7 Nurse.
· Contribute to case conferences and incident meetings.
· Identify clinical risks and ensure that appropriate action is taken, in accordance with the Trust Risk Management policy. Work in collaborative manner with other agencies in order to co-ordinate the care of patients and to prevent nosocomial spread of TB.
