We're excited to offer a fantastic opportunity for a skilled and compassionate Senior Practitioner/Senior Social Worker to join our dedicated Adult Social Care Team here at Wokingham Borough Council, on a full-time, permanent basis.
This role sits within our All Age Continuing Care and S117 Support Team, a specialist service supporting children and adults with complex health and care needs. Working closely with social care, health colleagues and partner organisations, the team helps to secure the right funding, coordinate assessments and make sure people receive the support they need.
