Our company is looking for a Social Worker to join our team.
Responsibilities:
- Carry out single assessments;
- Prepare Conference report;
- To be responsible for the assessment, planning, monitoring and review functions for all allocated children and families;
- Undertaking thorough and informed comprehensive assessments of need and risk;
- Undertaking investigations under Section 47s on allocated and duty cases as required;
- Attend any Court hearings as appropriate and give evidence when necessary;
- To promote a strengths based approach with local community solutions;
- To ensure each child has a detailed Care Plan which can be presented to Review, in consultation with the team manager;
- To promote placement stability and permanence for looked after children;
- To provide advice and development through supervision; practice educator roles; coaching and observing practice;
- Providing advice and support to Foster Carers;
- Undertaking pathway plans for Care Leavers;
- To ensure timescales are met for Court reports and panel papers when necessary;
- Undertaking complex assessment on Looked After Children;
- Supporting Looked Children with their LAC reviews.
Requirements:
- Experience within learning disabilities, older people or hospital discharge;
- Excellent case management skills;
- Professional Social Work qualification;
- HCPC Registration;
- knowledge of the CHC process;
- Dedicated time for quality reflective supervision and to learn from regular quality audits;
- Parking Permit;
- Agile working;
- Supportive management team and diverse caseload;
- Easy and regular access to the leadership team and principal social worker;
- Clear line management and practice development manager mentoring support.
